Adolesan Erkeklerde Sigara İçimi ile Sekonder Polisitemi Arasındaki İlişki

Özet

Amaç: Sigara önemli bir morbidite ve mortalite kaynağı olup, ergenler arasında sigara kullanımı gün geçtikçe artmaktadır. Sigara içimi ile dokulardaki oksijen oranı azalır, eritropoetin miktarında artış ve buna ikincil eritrosit seviyesi ve hemoglobin (hb) miktarında artış oluşur. Bu çalışmanın amacı ergenlik dönemi erkeklerde aktif ve pasif sigara maruziyetinin polisitemi üzerine etkisini belirlemektir. Gereç ve Yöntem: Çalışma Dr. Sami Ulus Kadın Doğum, Çocuk Sağlığı ve Hastalıkları Eğitim ve Araştırma Hastanesi Çocuk Hematoloji Polikliniği’ne polisitemi nedeni ile başvuran, 13-18 yaş arası, 98 erkek hastada, Ocak 2019-Ekim 2020 tarihleri arasında retrospektif olarak yapıldı. Olgular, aktif sigara içenler (23 hasta), pasif sigara maruziyeti olanlar (29 hasta) ve hiç sigara içmeyenler (46 hasta) olmak üzere üç gruba ayrıldı. Bulgular: Hastaların yaş ortalaması 15.9 ±1. 2 yıl (13-18 yıl) idi. Çalışma grubunda sigara içme sıklığı %23 olarak bulundu. S igara içen hastaların karboksihemoglobin değerleri anlamlı olarak yüksekti. Aktif sigara içenlerin hemoglobi n değerleri pasif sigara maruziyeti olanlar ve hiç içmeyenlerle karşılaştırıldığında anlamlı fark bulunmadı. Pasif sigara maruziyeti olanlarda eritropoetin düzeyleri, istatistiksel olarak anlamlı fark olmamasına rağmen diğer gruplardan daha yüksekti. Sonuç: Ergenlik döneminde polisitemi ile takip edilen hastalarda sigara maruziyet oranı yüksek sapta ndı (%53). Buda ergenlik döneminde aktif ve pasif sigara maruziyetinin önemli bir sekonder polisitemi nedeni olduğunu göstermektedir.

Abstract

Introduction: Smoking is an important cause of morbidity and mortality and is increasing among adolescents. Smoking decreases the oxygen supply to tissues, increasing erythropoietin causing excessive production of erythrocytes and elevated levels of hemoglobin. The aim of this study is to determine the effect of active and passive smoking on polycythemia in adolescents. Materials and Methods: This study was retrospectively performed between January 2019 and October 2020 in 98 male patients aged 13-18 years who applied to the Dr. Sami Ulus Maternity and Children's Health and Diseases Training and Research Hospital, Department of Pediatric Hematology for polycythemia. The cases were divided into three groups as active smokers (23 patients), passive smokers (29 patients) and never smokers (46 patients). Results: The mean age of the patients was 15.9 ±1.2 years (13- 18 years). The frequency of smoking in the study group was found to be 23%. Carboxyhemoglobin values of smoking patients were significantly higher. The hemoglobin values of active smokers were not found to be significantly different when compared with passive smokers and nonsmokers. Erythropoietin levels were higher in those with passive smoking exposure than in the other groups, although there was no statistically significant difference. Conclusion: The rate of smoking exposure was found to be high in patients who were followed up with polycythemia during adolescence (53%). This shows that active and passive smoking exposure during adolescence is an important cause of secondary polycythemia.
