Ankilozan spondilitli hastalarda hastalık aktivitesi ile depresyon, anksiyete ve yaşam kalitesi arasındaki ilişki

Özet
Amaç: Çalışmamızda ankilozan spondilitli (AS) hastalarda hastalık aktivitesi ile depresyon, anksiyete, yaşam kalitesi arasındaki ilişki araştırılmıştır. Yöntem: Çalışmaya 57 AS’li hasta ile 53 sağlıklı kontrol olmak üzere 110 kişi katıldı. Hasta ve kontrol grubunun depresyon ve anksiyete durumları, BECK Depresyon Envanteri (BDE) ve BECK Anksiyete Envanteri (BAE) ile değerlendirildi. Hastaların hastalık aktivitesi, fonksiyonel durumu ve yaşam kalitesi Bath Ankilozan Spondilit Hastalık Aktivite İndeksi (BASDAI), Bath Ankilozan Spondilit Hastalık Fonksiyonel İndeksi (BASFI), Ankilozan Spondilit Yaşam Kalitesi (ASQoL) ölçekleri ile değerlendirildi. Bulgular: Çalışmaya katılan AS’li hasta ve sağlıklı kontrol grubunun yaş ve cinsiyet dağılımları benzerdi. Ortalama depresyon ve anksiyete skorları AS grubunda kontrol grubuna göre daha yüksekti (sırasıyla AS’li grupta 14,89±10,23, 11,59±10,59, sağlıklı grupta 3,81±5,49, 2,04±3,08). AS grubunda BASDAI skoru düşük olanlar yüksek olanlara göre, daha düşük BDE skoru, BAE skoru ve ASQoL skoruna sahipti. HLA-B27 pozitif AS grubunda BDE skoru ve ASQoL skoru bakımından anlamlı bir fark yokken, BAE skoru HLA-B27 pozitif olan grupta daha yüksek ve istatistiksel olarak anlamlı bulundu (p
Abstract
Objective: The objective of this study is to investigate the relationship between disease activity and depression, anxiety and quality of life in ankylosing spondylitis (AS). Methods: Hundred and ten subjects, including 57 patients with AS and 53 healthy controls, participated in the study. Depression and anxiety levels of all participants were evaluated with the BECK Depression Inventory (BDI) and the BECK Anxiety Inventory (BAI). Disease activity was evaluated with Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), functional status with Bath Ankylosing Spondylitis Functional Index (BASFI), and quality of life with Ankylosing Spondylitis Quality of Life Questionnaire (ASQoL). Results: The age and gender distribution were similar between the groups. When the two groups were compared, the patient group had higher mean scores for depression and anxiety (for AS patients 14.89±10.23, 11.59±10.59; for healthy group 3.81±5.49, 2.04±3.08). AS patients with low BASDAI scores had lower BDI, BAI and ASQoL scores compared to those with high scores.
