Güney Marmara Bölgesi'nde 2011-2018 Yılları Arası Otopsisi Yapılan Bebek Ölümlerinin Retrospektif Değerlendirilmesi

Özet
Amaç: Güney Marmara Bölgesinde otopsi işlemi için Adli Tıp Kurumu Bursa Grup Başkanlığına gönderilen 0-24 aylık 474 bebeğin ölüm olgusu araştırılıp otopsi ve adli inceleme bulgularının literatür eşliğinde değerlendirilmesi amaçlanmıştır. Araçlar ve Yöntem: 2011-2018 yılları arasında otopsisi yapılan 474 bebek ölümleri olguları; cinsiyet, bebeğin öldüğü zamanki yaşı (gün olarak), ölüm orijini ve nedeni, ölüm yılı, ölüm yeri ya da ölü bulunduğu yer, ölümlerin mevsimsel dağılımı, patolojik ve toksikolojik inceleme sonuçları açısından değerlendirilmiştir. Veriler, kodlandıktan sonra bilgisayara yüklenmiş ve SPSS programı ile değerlendirilmiştir. Bulgular: 2011-2018 yılları arasında otopsisi yapılan 0-2 yaş grubu 474 çocuğun yaş ortancası 90 gün olarak elde edilmiştir. Çocukların %57,3’ü (n=271) erkek, %15,6’sı (n=74) Suriyeli uyruklu olduğu saptanmıştır. Ölüm nedeni en fazla akciğer hastalığı (%25,7) ve asfiksi (%14,3); ölüm orijini ise en fazla doğal ölüm (%52,7) şeklinde belirlenmiştir. Ölüm nedenleri aylara göre ele alındığında, Aralık, Ocak, Mart ve Mayıs aylarında yapılan otopsilerde ölüm nedeni en fazla akciğer hastalıkları olarak belirlenmiştir. Sonuç: Bu çalışma, Güney Marmara Bölgesi için bebek ölümlerinin oranı ve nedenleri açısından aydınlatıcı bir gösterge olabileceğinden önem kazanmaktadır. Bu bebek ölümlerine otopsi işlemi uygulanmamış olan diğer klinik çalışmalarla birlikte önlenebilir bebek ölüm nedenleri olan yabancı cisim aspirasyonu, enfeksiyon ve travmalar açısından dikkat çekilmesi gerektiğini belirtmiştir. Ayrıca bebek ölümlerinde otopsi işlemi uygulanması ve raporlama standartları konusunda bilgiler sunulmuştur.
Abstract
Introduction It was aimed to investigate the death cases of 474 infants aged 0-24 months who were sent to the Forensic Medicine Institute Bursa Group Presidency for autopsy in the Southern Marmara Region and to evaluate the autopsy and forensic examination findings in the light of the literature. Materials and Methods 474 infant mortality cases autopsied between 2011-2018 years; were evulated sex, age at death, origin and cause of death, year of death, place of death or place of death, seasonal distribution of deaths, pathological and toxicological examination results. Results 474 children aged 0-2 were autopsied in between 2011 and 2018 years, the median age was 90 days. It was determined that 57.3% (n=271) of the children were male and 15.6% (n=74) were Syrian nationals. The most common cause of death was lung disease (25.7%) and asphyxia (14.3%); the origin of death was determined as the most natural death. Considering the causes of death by months, the most common cause of death was lung diseases in autopsies performed in december, january, march and may. Conclusion This study gains importance as it can be an illuminating indicator for the rate and causes of infant mortality in the Southern Marmara Region. The most common causes of death in our study were preventable infant deaths such as lung infection, foreign body aspiration, and traumas; are the causes of death that should be considered by other clinics. It may be beneficial to provide breastfeeding education to expectant mothers and to increase their first aid knowledge. In addition, high mortality rates due to lung infection may be a guide for other clinics against infant mortality.
