Akut Posterior Multifokal Plakoid Pigment Epitelyopati'deKlinik ve OptikKoherens Tomografi Bulgular

Özet
Bu çalışmada göz hastalıkları polikliniğine görme azlığı şikayeti ile başvuran ve yapılan detaylı göz muayenesi sonucunda akut posterior multifokal plakoid pigment epitelyopati (APMPPE) tanısı konulan 7 olguya ait klinik, anjiyografik ve optik koherens tomografi (OKT) bulguları sunulmaktadır. Hastalarımızın beşi kadın ikisi erkekti, yaşları 15 ve 44 arasında değişmekteydi. Hastaların dördü akut, üçü remisyon döneminde başvurdu. Hastalar 5 gün ile 9 ay arasında değişik sürelerde takip edildi. Akut dönemde başvuran dört hastada subretinal mayi oluşumu, birinde ise ek olarak retina pigment epitel dekolmanı gelişti. görme keskinlikleri bize akut dönemde başvuran üç hastada 10/10 düzeyine ulaştı. Hastaların akut dönemde çekilen anjiyografilerinde lezyonlara uyan bölgelerde erken fazda hipofloresans, geç fazda hiperfloresans izlendi. Remisyon döneminde çekilen anjiyografilerde ise koroidal perfüzyonun etkilendiği alanlar hipofloresan, pencere defektinin olduğu alanlar hiperfloresan olarak izlendi. OKT görüntülemede ise akut hastalıkta koroidde kalınlaşma, foveada ödem ve dekolman, IS/OS (iç sement/dış segment) bandı ile RPE arasında hiperreflektans, IS/OS bandında ise harabiyet gözlendi. İlerleyen dönemlerde bu bulguların nispeten gerilediği ancak remisyon döneminde RPE ve koryokapillaris arasındaki düzensizliklerin sebat ettiği görüldü. APMPPE'li hastaların çoğunda nihai olarak görme keskinliğinde artış gözlenmektedir. Aynı şekilde akut dönemde OKT'de görülen bulguların çoğu klinikle uyumlu olarak gerilemekte, remisyon döneminde ise daha çok RPE düzensizliği görülmektedir
Abstract
In this study we present clinical, angiographic and optical coherence tomographic (OCT) findings of 7 patients who applied to ophhtalmology outpatient department with the complaint of decreased vision and were diagnosed as acute posterior multifocal placoid pigment epitheliopathy (APMPPE) after a detailed ophthalmological examination. Five of the patients were female and ages ranges from 15 to 44. Four of the patients were in acute period, three were in the remission period. Patients were followed up for 5 days to 9 months. Four patients in the acute period had subretinal fluid formation, one of them had pigment epithelium detachment (PED). In three acute phase patients visual acuity increased to 10/10. Angiograms of patients in the acute period showed multiple hypofluoresencent lesions in the early phase a hyperfluorescence in the late phase. In remission period angiography showed hypofluorescent areas due to affected choroidal perfusion, and hyperfluorescent areas due to window defect. OCT images demonstrated serous retinal detachment, retinal edema, hyperreflectance between inner segment/outer segment (IS/OS) junction and RPE and damage in IS/OS junction in acute period. In later periods, these findings were relatively resolved but in the remission period, irregularities between RPE and choriocapillaris were well defined. In the majority of patients with APMPPE finally there is an increase in visual acuity. Most of OCT findings in the acute phase are resolved in accordance with clinical improvement, also OCT reveals RPE irregularities in remission period
