Percutaneous closure of small to moderate ventricular septal defects: Early clinical and procedural results

Özet

Amaç: Bu çalışmada, Amplatzer Septal Okluder ile perkütan Ventriküler Septal Deefekt kapatılmasıyla ilgili klinik ve işlmle ilgili sonuçlarını sunmayı amaçladık. Yöntem ve Gereçler: 2008-2011 yılları arasında, 5 kardiyoloji merkezinde, 23 hastaya, Amplatzer Septal Okluder kullanılarak başarılı Ventriküler Septal Defekt perkütan kapama işlemi uygulandı. Hastalar işlemden bir ay sonra klinik ve ekokardiyografik olarak değerlendirldi. Floroskopi süresi, işlem süresi, pulmoner şant oranı, ekokardiyografik olarak Ventriküler Septal Defekt çapları, kullanılan cihaz tipleri ve hemodinamik veriler kaydedildi. Bulgular: Hastaların tümünde işlem başarılı oldu. Floroskopi süresi 25±20 (15-112) dakika, işlem süresi 90±51 (40–145) dakikaydı. İşlem sırasında ölüm veya acil cerrahi gereksinim gerektiren komplikasyon görülmedi. Bir hastada cihaz embolizasyonu, bir hastada 3. derece atrioventriküler blok, 3 hastada cihaz içinden rezidü şant ve 3 hastada kasık bölgesinde hemotam varlığı kaydedildi. Hastaların takibinde ölüm, endokardit veya cihazla ilgili bir olumsuz olay kaydedilmedi. Bir hastada rezidü şant devam etti. Sonuç: Perkütan yolla Amplatzer Septal Okluder kullanılarak Ventriküler Septal Defekt kapatılması çok iyi işlemsel ve klinik sonuçlarla birlikte, uygulanabilir ve güvenli bir tekniktir.

Abstract

Aim: In this study, we aimed to present the procedural and early clinical results percutaneous closure of Ventricular Septal Defects with Amplatzer septal occluders Materail And Methods: Between 2008 and 2011, 23 patients were underwent successfully percutaneous Ventricular Septal Defect closure in 4 cardiology centers. Percutaneous closure was performed with Amplatzer Septal Occluders. Patients were evaluated clinically and by echocardiography one month after the procedure. fluoroscopic time, procedure duration, pulmonary/systemic shunt ration, Ventricular Septal Defect diameter on echocardiography, types of devices used and hemodynamic data were all recorded. Results: Percutaneous closures of Ventricular Septal Defects were successfully performed in all patients. Fluoroscopic time (min) was 25±20 minutes (range 15–112), procedure time (min) was 90±51 (range 40–145). There was not any death or urgent cardiac surgery required complication. There was one device embolization, one third degree atrioventricular, intra device residual shunt n 3 patients for 3 days and mild groin hematoma was occurred in three patients. There was not any significant adverse event in patients group during follow-up including death, endocarditis or device related complication. Residual shunt was remained in one patient. Conclusion: Percutaneous closure of Ventricular Septal Defects with Amplatzer septal occluders is a feasible and safe technique with excellent early procedural and clinical results.
