Pankreas adenokarsinomunda hepatit prevelansı

Özet
Amaç: Hepatit (B ve C) bizim ülkemiz için de ciddi bir problemdir. (HBsAg) için seropozitifik oranı %0,1-0,5 ikenanti-hepatit B antikoru (Anti-HBs) için %5-10dur. Vaka kontrollü bir çalışmada, Türk kanserli hastalarda seropo- zitifik oranları hepatit B yüzey antijeni (HBsAg) için %4,8, hepatit B antikoru (anti-HBs) için %14 ve anti-hepatitC antikoru (anti-HCV) için %2.8 olarak saptanmıştır. Lösemi, lenfoma gibi bazı maliynitelerde daha yüksek orandaHBsAg ve anti-HCV seropozifik oranı beklenir. Ancak, pankreas kanserinde bu durum net değildir.Gereç ve Yöntem: Farklı merkezlerde takip edilen pankreas adenokarsinomlu 399 hasta retrospektif olarak değerlen- dirildiğinde 310 hastanın tanı sırasındaki seropozitifik verilerine ulaşılabilmiştir. Seropozitifikenzyme-linked im- munosorbent assay (ELISA) ile değerlendirilmiştir.Sonuç: Tüm hastalar için erkek / kadın oranı 1.8 (257/142), medyan yaş 61 (24-89) idi. Medyan yaşlar HBsAg pozi- tif olanlarda 58, anti-HBs pozitif olanlar için 60 ve anti-HCV pozitif olanlar için ise 65 olarak saptanmıştır.Seropozi- tifik oranları ise HBsAg için %5.8, anti-HBs için %8.2 ve anti-HCV için %1.9 idi. Hastaların hiçbirinde HBsAg veanti-HCV birlikte pozitif değildi.. Yine, anti-HCV pozitif hastalarda HBsAg veya anti-HBs serpozitif değildi. Tartışma: Seropozitifik oranları kanserli hastalarla benzer görünmektedir. Tedavi öncesinde, özellikle premedikas- yonda bile steroid içeren tedavilerden önce tanı sırasında hepatit belirteçlerinin bilinmesinin önemi her zaman akıl- da tutulmalıdır.
Abstract
Aim: Hepatitis (B and C) is a serious problem almost for our country. Seropositivity rates were determined as 4.8%for hepatitis B surface antigen (HBsAg), 14% for anti-hepatitis B (anti-HBs) and 2.8% for anti-hepatitis C (anti- HCV) in Turkish cancer patients, in a case-control study. Some malignancies like leukemia and lymphoma have ahigher trend for positive seropositivity for HBsAg and anti-HCV. However, there is no data about pancreatic carci- noma. Our aim was to evaluate hepatitis prevalence in pancreatic adenocarcinoma.Material and Methods: 399 pancreatic adenocarcinoma cancer patients followed-up at different centers were evalu- ated retrospectively. We could have reached only 310 patients seropositivity data at diagnosis. Seropositivity wasanalyzed with enzyme-linked immunosorbent assay. Results: Male/female ratio was 1.8 (257/142) for all patients. Median age was 61 (range=24-89) for all group. Fur -thermore, median ages for subgroups were as 58 for HBsAg, 60 for anti-HBS and 65 for anti-HCV seropositivepatients. Seropositivity rates were determined as 5.8% for HBsAg, 8.2% for anti-HBs and 1.9% for anti-HCV. Noneof the patients had positive seropositivity for both HBsAg and anti-HCV. Also, anti-HCV seropositive patients hadneither HBsAg nor anti-HBs seropositivity.Discussion: The seropositivity rates seem to be similar to those in cancer patients. The clinical importance of deter-mining hepatitis markers at diagnosis before scheduling treatment modality, especially those involving steroid evenas premedication should be kept in mind.
