Klozapin kullanan hipersalivasyonlu hastalarda submandibuler glanda botulinum nörotoksin uygulaması

Özet
Amaç: Klozapin şizofreni tedavisinde; klasik nöroleptiklere cevap vermeyen, intolerans geliştiren veya tedaviye di-rençli vakalarda kullanılan antipsikotik bir ilaç olup en önemli yan etkilerinden birisi salivasyon artışıdır. Bu çalışma-da klozapin kullanan hastalarda salivasyon artışının engellenmesi için kullanılan botulinum toksin enjeksiyonununsalivasyon miktarları üzerindeki etkileri ve bunun hastaların yaşam kalitesi üzerindeki etkisi araştırılmıştır.Gereç ve Yöntem: Çalışmamızda klozapin kullanan üç hastanın botulinum toksin enjeksiyonu öncesi salivasyonmiktarları ölçülmüş ve salivasyon dereceleri skorlanmıştır. Elde edilen veriler botulinum toksini enjeksiyonu sonra-sı değerlerle karşılaştırılmıştır.Bulgular: Çalışmaya katılan hastaların enjeksiyon öncesi salivasyon miktarları sırası ile; 1,8 ml, 1,2 ml ve 1,4 mliken enjeksiyon sonrası salivasyon miktarları 0,2 ml, 0,1 ml ve 0,2 ml olarak ölçüldü. Hastaların salivasyon miktar-larında %88, %91, %85 oranında azalma tespit edildi. Tüm hastaların salivasyon derecelerinde belirgin şekilde azal-ma görüldü.Sonuç: Hipersalivasyon tedavisinde submandibuler glanda botulinum toksin enjeksiyonu oldukça etkili bir tedaviyöntemi olup özellikle etkilerinin geçici olması nedeniyle kullanımı önermekteyiz.
Abstract
Aim: Clozapine is an antipsychotic drug which is used to treat severe schizophrenia or in cases where patient doesnot respond to conventional neuroleptics. One of the most important side effects of this drug is increase in salivation.In this study we investigated the effects of botulinum toxin injection which is used in order to prevent the salivationincrease, and the effects on the quality of life of the patients who use clozapine. Material and Method: In this study the salivation amounts of three patients who use clozapine were measured, be- fore and after the botulinum toxin injection and compared. Results: The salivation amounts of three patients were measured as 1,8 ml, 1,2 ml ve 1,4 ml respectively before theinjection, and as 0,2 ml, 0,1 ml and 0,2 ml after the injection. 88%, 91% and 85 % reduction was detected in thesalivation amounts of the patients. All patients salivation degrees decreased signifcantly.Conclusion: Botulinum toxin injection to the submandibular gland is a very effective treatment method for hyper- salivation and it is also recommended since it has temporary effects.
