İleri Yaşlı Hastalarda Kalça Artroplastisi İçin Uygulanan Farklı Anestezi Yöntemlerinin Retrospektif Olarak Değerlendirilmesi

Özet
Amaç: Yaşlı hastalarda anestezi, yaşlanmaya bağlı oluşan fizyolojik ve psikolojik değişimleri içine alacak şekildespesifik bir yaklaşımı gerektirir. Bu çalışmada kalça artroplastisi uygulanan yaşlı hastalarda farklı anestezi tiplerininperoperatif komplikasyonlar, morbidite ve mortalite üzerine olan etkileri değerlendirildi. Yöntem ve Gereçler: Bu çalışmada, 2008-2010 yılları arasında kalça artroplastisi operasyonu geçiren 80 yaş üzeri157 hasta retrospektif olarak analiz edilmiştir. Uygulanan anestezi tekniğine göre hastalar üç gruba ayrıldı: Grup I:genel anestezi (n=39), Grup II: nöroaksiyel anestezi (n=94), Grup III: periferik sinir blokajı (n=24). ASA (AmericanSociety of Anesthesiologists) sınıflaması, demografik bulguları, ek hastalıkları, operasyon süresi, intraopratif kanihtiyacı, pre-ve postoperatif hemoglobin değerleri, peroperatif komplikasyonlar, postoperatif yoğun bakım ünitesi(YBÜ) ihtiyacı ve YBÜde ve hastanede kalış süreleri, mortalite ( 7.-30. gün) değerlendirildi. Bulgular: Hastaların cerrahi süreleri ve hipotansiyon sıklığı Grup I de diğer gruplara göre daha az bulundu (p:0,021,p:0,004). Grup II deki hastaların hastanede kalış sürelerinin, Grup Ie göre daha kısa olduğu görüldü (p=0,015). GrupIIIde ilk 30 gündeki mortalite diğer gruplara göre daha yüksek bulundu (p=0,009).Sonuç: Bulgularımız yaşlı hastalarda anestezi tekniği seçilirken hastanın klinik özellikleri dikkate alındığı takdirdehiçbir yöntemin diğerine üstün olmadığını göstermiştir.
Abstract
Aim: Anesthesia in the elderly requires a specific approach including physiological and psychological alterations dueto the aging process. In this study evaluated the effects of anesthesia type on peroperative complications, mortalityand morbidity in elderly patients undergoing hip arthroplasty.Material And Methods: In this study, one hundred fifty-seven patients older than 80 years who were operated for hiparthroplasty between 2008-2010 were retrospectively analyzed. Patients were divided into three groups according theanesthesia techniques applied: general anesthesia Group I (n=21), neuraxial block Group II (n=37), and peripheralnerve blockade Group III (n=51). ASA (American Society of Anesthesiologists) classification, the demographic find- ings, additional diseases, operative time, blood needed surgeries, pre-and postoperative hemoglobin values, periop- erative complications, postoperative intensive care unit (ICU) need and length of stay ICU and hospital, mortality (7and 30 days) were evaluated. Results: The duration of surgery and the frequency of hypotension were significantly lower in Group I comparedto others (p:0,021, p:0,004). The length of hospital stay was significantly longer in Group I compared to Group II(p:0,015). Mortality in the first 30 days in Group III were higher than the other groups (p:0,009). Conclusion: Our results show that no technique was superior to the others if the anesthetic technique selected accord- ing to the clinical features of them.
