Özet

Amaç: Ocak 2008- Aralık 2009 yılları arasında çeşitli kliniklerden tüberküloz (tbc) laboratuarına gönderilen örneklerden hazırlanan boyalı preparat ve mikobakteri kültür sonuçları retrospektif olarak değerlendirildi. Yöntem ve Gereçler: Mikobakteriyolojik inceleme için gönderilen toplam 572 örnek hem EZN hem de mikobakteri kültürü yapılarak değerlendirildi. Kültür için, tüm örnekler Löwenstein-Jensen (LJ) ve BacT/ALERT 3D şişelerinin her ikisine birden ekildi. Bulgular: EZN boyama yönteminin duyarlılığı %47, özgüllüğü %98 olarak bulundu. Kültür yöntemlerinden LJ’nin pozitiflik saptama oranı %81, BacT/ALERT 3D’nin ise %100 bulunurken, mikobakteri saptama hızları LJ için ortalama 20,5 gün, BacT/ALERT 3D sıvı kültür sisteminde ise 12,5 gün olarak saptandı. Sonuç: Tüberküloz laboratuarımızda yapılan EZN boyama yönteminin duyarlılığı diğer çalışmalarla uyumlu olarak düşük saptanmıştır. Diğer yandan otomatize kültür sistemi BacT/ALERT 3D’nin, geleneksel kültür yöntemi olan LJ ile kıyaslandığında, hızlı ve güvenilir olduğu görülmüştür. Ayrıca tüberkülozun doğru tanısı için EZN boyamanın tek başına yeterli olmadığı ve mutlaka kültür ile birlikte değerlendirilmesi gerektiği sonucuna varılmıştır.

Abstract

Objective: In this study, stained preparation and tuberculosis (tbc) culture results from specimens that were sent from various clinics from January 2008 to December 2010 were evaluated retrospectively. Material and Methods: A total of 572 samples were evaluated with Ehrlich-Ziehl-Neelsen (EZN)and mycobacteria cultures. The specimens were inoculated on both of the Lowenstein-Jensen (LJ) and BacT/ALERT 3D bottles for culturing. Results: The sensitivity and specificity of EZN staining method were 47% and 98%, respectively. Positive detection rates of LJ and BacT/ALERT 3D were found as 81%, and 100%, respectively. Additionally, average detection period of tbc bacilli for LJ was 20,5 days whereas it was 12,5 days for BacT/ALERT 3D liquid culture system. Conclusion: In this study the sensitivity of the EZN staining method was detected to be low in concordance with other studies. On the other hand, the automated BacT/ALERT 3D tbc culture system was rapid and reliable compared to traditional LJ culture method. Additionally, EZN staining method alone was not enough for accurate diagnosis of tuberculosis and it certainly should be supported with culture methods.
