Hastane Kaynaklı Enfeksiyonlardan İzole Edilen E. coli ve Klebsiella Türü Gram Negatif Basillerin İmipenem, Meropenem, Ertapenem, Piperasilin- Tazobaktam Duyarlılıklarının E-Test Yöntemiyle Belirlenmesi

Özet
Amaç: Bu çalışmanın amacı, Ankara Eğitim ve Araştırma Hastanesinde Hastanesinde Nisan 2007- Ocak 2008 yıllarıarasında yatan hastaların çeşitli klinik örneklerinden izole edilen ve hastane enfeksiyonu etkeni olan 67 E.coli ve 33Klebsiella türü (spp.) bakterinin karbapenem grubu antibiyotikler [imipenem (IMP), meropenem (MEM) , ertapenem(ETP) ] ve piperasilin-tazobaktam (TZP) duyarlılıklarının belirlenmesiydi.Yöntem ve Gereçler: Çalışmaya 48 genişlemiş spektrumlu beta-laktamaz üreten (GSBL, ESBL) E.coli, 19 GSBLüretmeyen E.coli, 17 GSBL üreten Klebsiella spp., ve 16 GSBL üretmeyen Klebsiella spp. olmak üzere toplam100 suş dahil edildi. E.coli ve Klebsiella spp. suşlarında GSBL varlığı çift disk sinerji ve E-test yöntemleriylebelirlendi. Suşların IMP, MEM, ETP ve TZP antibiyotikleri için minimal inhibitör konsantrasyon (MİK) değerleriüretici firmanın önerileri doğrultusunda ticari E-test stripleri kullanılarak saptandı. Antibiyotik duyarlılıkları ise CLSIönerileri doğrultusunda belirlendi. Bulgular: Tüm E.coli ve Klebsiella spp. suşlarının tamamı IMP (%100) , MEM (%100) ve ETP (%100)e duyarlıidi. GSBL üreten (pozitif) ve GSBL üretmeyen (negatif) E.coli suşlarının TZPe duyarlılık oranları sırasıyla; %94 ve%100; GSBL pozitif ve GSBL negatif Klebsiella spp. suşlarının TZPe duyarlılık oranları ise sırasıyla; %59 ve %94olarak belirlendi. Sonuç: Ülkemizde kullanıma yeni giren antibiyotiklerden ertapenemin, GSBL üreten E. coli ve Klebsiella spp.suşlarına karşı imipenem ve meropenem kadar oldukça etkili olduğu saptandı. Nozokomiyal enfeksiyonlarda akılcı ampirik antibiyotik tedavisi enfeksiyon etkeni olan patojenlere ait karakteristiksürveyans verileri (GSBL üretimi vb.) ve antibiyotik duyarlılık oranları göz önüne alınarak planlanmalıdır.
Abstract
Aim : The aim of this prospective study was to determine the MIC value of of 67 E.coli and 33 Klebsiella strainsagainst to carbepenems [ imipenem (IMP) , meropenem (MEM) , ertapenem (ETP) ], and piperacilline-tazobactam(TZP) isolated from nosocomial infection at Ankara Education and Research Hospital in 2007-2008 .Material and Method: Totally 100 strains, including 48 ESBL positive E.coli, 19 ESBL negative E.coli, 17 ESBLpositive Klebsiella spp., and 16 ESBL negative Klebsiella spp. isolated from clinical materials in hospitalized pa- tients were included in study. Presence of ESBL-producing strains of E.coli and Klebsiella spp. were determined by using double-disk synergyand E-test methods according to CLSI. The MIC values of strains for IMP, MEM , ETP, TZP were determined byusing commercial E-test strips according to manufacturer . Antibiotic susceptibilities were determined according toCLSI recommendations.Results : All the E.coli and Klebsiella spp. strains were found susceptible to IMP (%100), MEM (%100) , ETP(%100). The susceptibility rate for ESBL positive and negative E.coli strains to TZP was %94, %100, respectively.The susceptibility rates for ESBL positive and negative Klebsiella spp. strains were determined to be %59, %94 forTZP, respectively.Conclusion: In present study, ertapenem was found to be highly effective as imipenem and meropenem against tostrains of ESBL pruducing E.coli and Klebsiella spp. Rational antimicrobial treatment of nosocomial infectionsshould be adjustment according to nosocomial infection surveillance dates related to the prevalence of charecteristicspathogens ( such as ESBL producing and non producing, multidrug resistance) and antimicrobial susceptibility ratesof the hospital.
