Penil erektil fonksiyon her hekim tarafından sorgulanmalı mıdır?

Özet
Toplumsal ve sosyokültürel farklılıklar hastaların cinsel fonksiyonlar ile ilgili sorunlarını dile getirmesini güçleştir- mektedir. Gerek utanmaları gerekse erektil disfonksiyonu yaşlanmanın doğal bir sonucu olarak kabul etmeleri nede- niyle hastalar mevcut şikayetlerini kolayca ifade edemezler. Ayrıca erektil disfonksiyonun yaygın olduğunu ve teda- visinin büyük oranda mümkün olduğunu bilmezler. Tüm bu sebeplerle hastalar cinsel işlevler ile ilgili sorunlarının hekimler tarafından fark edilmesini ve sorgulanmasını beklerler. Hekimler de sorun odaklı çalışma tarzlarından dola- yı cinsel işlevler ile ilgili sorunları çoğunlukla sorgulamamaktadırlar. Bunun yanında bazı hekimler erektil disfonk- siyonun yüksek morbidite ve mortaliteye sahip diğer hastalıklara göre daha az önemli olduğunu düşünürler. Oysaki erektil disfonksiyon diyabet, koroner arter hastalığı, depresyon gibi diğer ciddi hastalıkların bir komponenti ve hatta ilk bulgusu olabilir. Ayrıca erektil disfonksiyon her yaşta görülebilen bir sorundur. Yapılan çalışmalarda 40 yaş üstü erkeklerde yaklaşık %52-69, 40 yaş altı erkeklerde yaklaşık %25-30 düzeylerinde saptanmıştır. Her yaşta görülebil- mesi, ciddi komorbiditeye sahip hastalıklar ile yakın ilişkisi, her iki cinsiyeti etkilemesi, kolay tanı konulabilir ve te- davisinin büyük oranda mümkün olması nedeni ile erektil disfonksiyon başta aile hekimleri olmak üzere tüm hekim- ler tarafından sorgulanması gereken önemli bir sağlık sorunudur.
Abstract
Social and socio-cultural differences make it difficult to articulate issues related to sexual functions of patients. Due to both shame and acceptance of the situation as a natural consequence of aging, erectile dysfunction patients do not express their complaints easily. In addition, the patients do not know that erectile dysfunction is a common problem and it is possible to treat it to a large extent. For all these reasons, the patients expect their problems related to sexu- al functioning to be noticed and questioned by doctors. Doctors also tend not to question the problems with sexual functioning because of their problem-oriented working style. Besides, some doctors think that erectile dysfunction is less important than the other diseases having high morbidity and mortality. However, erectile dysfunction may be the first sign or even a component of the other serious diseases such as diabetes, coronary artery disease and depres- sion. In addition, erectile dysfunction is a problem that can be seen at any age. Studies demonstrated that 52-69% of men over 40 and 25-30% of men under 40 years of age have erectile dysfunction. Erectile dysfunction is a major he- alth problem and should be questioned by all physicians including family physicians because it is an easy-to be diag- nosed disease and treatable to a large extent, can be seen at any age, has a close relationship with the other diseases having serious comorbidity and affects both sexes.
