Rotator kılıf ve biseps tendon patolojilerinin değerlendirilmesinde ultrasonografi ve manyetik rezonans görüntüleme bulgularının karşılaştırılması

Özet

Amaç: Rotator kılıf ve biseps kası tendonu patolojilerinin tanısında, ultrasonografi (US) yönteminin tanı değerini manyetik rezonans görüntüleme (MRG) yöntemi ile karşılaştırmaktır. Yöntem ve Gereçler: Omuz ağrısı şikayeti olanlarda omuz US ve MRG incelemesi yapıldı. Toplam 105 olguda, iki yöntemin sonuçları retrospektif olarak değerlendirildi. Manyetik rezonans görüntüleme yöntemi bulguları standart olarak kabul edildi. Bulgular: Ultrasonografi ile biseps kası tendonu için, 8 dislokasyon olgusunun 7’ si, 3 parsiyel rüptürün 1’ i, 4 tam kat rüptürün ise tamamına doğru tanı konuldu. Biseps kası tendon patolojilerinde US’ nin duyarlılığı %80, özgüllü- ğü %97.8 idi. Supraspinatus tendon patolojileri için ise, 26 tam kat rüptürün 24’ ü (%92.3 duyarlılık, %98.8 özgül- lük); 37 parsiyel rüptürün 30’ u (%81 duyarlılık, %95.6 özgüllük); 34 tendinozisin 30’ u (% 83.3 duyarlılık, %95.6 özgüllük) US tetkiki ile doğru tanımlandı. Sonuç: Sonuç olarak, US incelemesi omuz ağrısı olan olgularda patolojilerinin değerlendirilmesinde ve tanısında gü- venilir, etkin ve tekrarlanabilir bir yöntemdir. Ancak parsiyel rotator kılıf rüptürü ve intrakapsüler biseps tendonunun parsiyel rüptür patolojilerinde tanı değeri düşüktür.

Abstract

Aim: Comparison of the diagnostic value of ultrasonography (US) and magnetic resonance imaging (MRI) in the diagnosis of rotator cuff and biceps tendon pathologies. Material and methods: Shoulder US an MRI were performed in patients suffering from shoulder pain. The results of the two methods were retrospectively evaluated in a total of 105 cases. Magnetic resonance imaging findins were accepted as a reference. Results: In the biceps tendon, ultrasonography served as a method for correct diagnosis in 7 of the 8 cases with dislocation, in 1 of 3 cases with partial rupture, and i all 4 cases with full thickness rupture. In biceps tendon pathologies, the sensitivity nad the specifity of US were respectively 80% and 978%. And for supraspinatus tendon patholo- gies; 24 of 26 full thickness ruptures (92.3% sensitivity, 98.8% specifity), 30 of 37 partial ruptures (81% sensitivity, 95.6% specifity), 30 of 37 tendnosis’ (83.3% sensitivity, 95.6% speifity) were diagnosed correctly with US. Conslusion: As a result, US is a reliable and especially reproducible method for evaluation and diagnosing of pa- tients with shoulder pain. Furthermore, its diagnostic value is low in partial rotator cuff ruptures and in the partial ruptures of the intracapsular biceps tendon.
