İdrar Kontaminasyonlarında Aynı Hastadan Yeniden Örnek Geliyormu?

Özet

Amaç: İdrar kültürü değerlendirmesinde genel olarak ≥3 üropatojen saptandıysa idrar kontaminasyonundan bahsedilir.Böyle durumlarda hastadan yeniden idrar örneği istenir.Fakat çeşitli nedenlerle bu örnek yeniden laboratuvaragelmemektedir. Bu çalışma 6 aylık süreç içerisinde kontamine olarak kabul edilen örneklerin laboratuvara yenidengelip gelmediğini ortaya koymak ve muhtemel nedenlerini tartışmaya açmak amacıyla yapılmıştır.Yöntem ve Gereçler: Bu amaçla otomasyon sisteminden geriye dönük olarak ilk önce kontamine olan idrarörnekleri taranmış ve bu örneklerin yeniden laboratuvara gelip gelmediği yaş, cinsiyet ve kliniklere göre dağılımıyapıldı. İstatistik yöntem olarak Z testi ile oran karşılaştırması yapıldı. Bulgular: Bu dönem içinde 1093 idrar numunesine kontaminasyon tanısı konmuştur. Kadın erkek oranı eşit olup,kontaminasyon dağılımı en sık 0-12 ay(314 hasta) yaş grubundadır. Kontrole gelme açısından en sık Çocuk NefrolojisiPolikliniği, gelmeme açısındanda Çocuk Acil ön sıraya geçmiştir.Sonuç: Özellikle idrar kültür istemi sırasında hekimler tarafından kültür antibiyogram sonucunu beklemedenhastaya verilen ampirik tedaviler sonrası ve Çocuk Acil gibi akut şikayetlerle başvurulup aynı anda istem yapılanama kontrol yeri poliklinikler olan bölümler de gelmeme oranı yükselmektedir. Bunun yanında özellikle idrar yolusemptomları ile başvuran hastalarda bu şikayetler önemsenerek bu tür durumlarda yeniden idrar örneğinin kültüramacıyla laboratuvara gönderildiği, bununda gelme oranını yükselttiğini düşündürmektedir.

Abstract

Aim: Urine contamination is suspected in the urine cultures presenting with ≥3 uropathogens. To confirm thissuspicion, a secondary urine sample is required for further analysis. However, some of the secondary samples are notdelivered to the laboratory due to various reasons. The aim of this study was to investigate whether the secondarysamples were delivered to the laboratory and also to find out the reasons as to why the secondary samples were notdelivered to the laboratory, over a six-month period.Material and Methods: Using the hospital database, the urine samples suspected with urine contamination werefound and then the delivery status of the secondary samples were categorized according to age, gender, and thedepartment requesting the secondary sample. As statistical methods, rate comparison was made with the Z test.Results: A total of 1,093 urine samples diagnosed with urine contamination were found. Gender distribution wasalmost the same, and urine contamination was mostly seen in patients aged 0-12 months (n=314). The highest rateof delivery was found in the Pediatric Nephrology Polyclinic and the highest rate of non-delivery was found in thePediatric Emergency Service.Conclusion: The highest rates of non-delivery were found in the patients who were prescribed empirical therapyby their physicians before obtaining the culture antibiogram results and in the departments admitting acute patientssuch as Pediatric Emergency Service, mainly because the secondary samples are requested by these departments butthe follow-up procedures of the patients are performed at the relevant polyclinics. In contrast, the highest rates ofdelivery were detected in the patients presenting with urethral syndrome symptoms, likely because these patients paysignificant attention to their health condition and thus deliver their secondary samples as required.
