Yoğun Bakım Ünitesinde Takip Edilen İntoksikasyon Olgularının Retrospektif Olarak İncelenmesi

Özet
Amaç: Dört yıl boyunca yoğun bakım ünitesinde takip edilen intoksikasyon olgularını, nedenleri ve prognozlarıaçısından retrospektif olarak değerlendirmeyi amaçladık. Yöntem ve Gereçler: Yoğun bakım ünitesinde Nisan 2009 Nisan 2013 tarihleri arasında takip edilen 130 intoksikas- yon olgusu retrospektif olarak incelendi. Olgular, yaş, cins, oluş şekli, önceki psikiyatrik hastalık ve kullandığı ilaçöyküsü, tedavi planları ve prognozları açısından değerlendirildi. Bulgular: İntoksikasyonla gelen olgular etyolojik açıdan incelendiğinde; 105 (80,3%) olgu ilaç, 6 (5%) olgu organo- fosfat, 3(2%) olgu karbonmonoksit, 1 olgu mantar (0,7%), 15(12%) olgu da diğer intoksikasyonlardı. (Şekil-1). Suisitamacıyla ilaç alımlarının 39%unu antidepresan ve antipsikotikler oluşturmaktaydı (Şekil-2). Hastaların 29%ununise psikiyatrik ilaç kullanım, 32%sinin psikiyatrik hastalık öyküsü mevcuttu (Şekil 3). Mekanik ventilatör ihtiyacıolan 5 (4%) hasta da ilaç intoksikasyonuna bağlı suisid olgusuydu. 100 (77%) hasta şifa ile taburcu olurken, 17si(13%) gözlem süresini tamamlamadan kendi istekleriyle taburcu edildi.12 (9%)si yatarak tedavi amaçlı psikiyatriservisine sevk edildi.Sonuç: Antidepresan ilaçların, mekanik ventilatör desteğine yol açabilen, yüksek mortalite ve morbiditeye sahipintoksikasyonlara yol açabileceği akılda tutulmalıdır.
Abstract
Aim: Intoxication cases monitored in the intensive care unit for four years have been retrospectively investigated interms of their causes and prognoses. Material and Methods: One hundred and thirty intoxication cases monitored in an intensive care unit between April2009 and April 2013 have been analysed retrospectively. Cases were handled in terms of age and gender, symptoms,previous psychiatric problems and drug history, treatment plans, and prognoses of the patients. Results: The primary causes were determined to be drug intake, organophosphate, carbon monoxide, and fungus in105 (80.3%), 6 (5%), 3 (2%), and 1 (0.7%) cases respectively, as seen in Figure 1. Fifteen (12%) of the patients werepoisoned by other agents. Drugs taken orally with a suicide attempt were mainly antidepressants and antipsychotics(Figure 2). Thirty-two percent of the patients had previous records of psychiatric illnesses, 29% of whom had psychi- atric drug prescriptions (Figure 3).Mechanic ventilation was required for five patients, all of whom had attempted suicide. One hundred patients (77%)were discharged from the hospital; 17 patients were discharged with self-consent after the completion of the obliga- tory monitoring period. Twelve patients were referred to the Psychiatry Unit and hospitalized.Conclusion: It has been proven that antidepressants can lead to high mortality and morbidity intoxications requiringmechanical ventilation.
