Hipermetropik astigmat nedeniyle LASEK girişimi yapılan hastada açıklanamayan refraktif sonuç

Özet
EBu sunumda LASEK yapılan bir hipermetrop astigmat olgusunda geç dönemde ortaya çıkan ve açıklama getirilemeyen bir komplikasyonun sunulması amaçlanmıştır. Nisan 2011 tarihinde gözlükten kurtulma talebiyle kliniğimize müracaat eden 25 yaşındaki bayan hastanın yapılan muayenesinde sağ gözde tashihsiz görmesi 0,1, +5,00 +1,00 55 tashihle 0,7; sol gözde ise tashihsiz görmesi 0,1, +4,75 +0,75 105 tashihle 0,6 olarak bulundu ve hastaya LASEK girişimi uygulandı. Hastada intraoperatif veya erken postoperatif herhangi bir komplikasyona rastlanmadı. Yaklaşık 2,5 yıl sonra hastanın sağ gözdeki görmesi 0,5, sol gözdeki görmesi 2 mps düzeyine gerilemişti. Otorefraktometre değerleri olan +1,00 +0,25 163/ +0,50 10 değerleriyle de görme artmıyordu. Hastaya korneal topografi, optik koherens tomografi ve kranial manyetik rezonans görüntüleme tetkikleri yapıldı, ancak açıklayıcı bir bulgu görülemedi. Son yapılan tashihinde sağ gözünde -0,75 camla 0,6, sol gözünde yine -0,75 camla 0,6 görme değerleri tespit edildi. Hasta halen bu değerlerde yazılan gözlüğü sorunsuz olarak kullanmaktadır. Refraktif girişimlerin sonucunda cihazlarla elde edilen değerlerin dışında refraktif sorunlarla karşılaşılabileceği akıldan çıkarılmamalıdır
Abstract
In this report it is aimed to introduce an unexpected and unexplainable complication of LASEK seen in the late postoperative period in a case of hyperopic astigmatism. A female patient, 25 year-old, was admitted to our clinic to get rid of her eyeglasses in April 2011. In her examination uncorrected visual acuity was 0.1 with snellen scale in her right eye and 0.1 in her left eye. Her best corrected visual acuity was found to be 0.7 with a correction of +5.00 +1.00 55 in her right eye and 0.6 with a correction of +4.75 +0.75 105. LASEK procedure was performed on her both eyes. No intraoperative or early postoperative complication was recorded. Uncorrected visual acuity decreased to 0.5 in her right eye and counting fingers from 2 m in her left eye at the end of aproximately 2.5 years. Her best corrected visual acuity was the same although there was a refractive error of +1.00 +0.25 163 in the right eye and a +0.50 10 in the left eye. Corneal topography, optical coherence tomography and cranial magnetic resonance were performed, but no pathology could be found. The refraction was repeated at the end and the patient succeeded to be able see 0.6 in each eye with a correction of -0.75. The patient still uses the eyeglass with these values without any complaint. Refractive problems other than those detected with the refractometers can be recorded after the refractive procedures
