Maternal effects of general and spinal anesthesia in cesarean sections

Özet

Amaç: Sezaryen operasyonlarında anestezi seçimi önem- lidir. Anne ve fetusun sağlığının devam ettirilebilmesi için en uygun yöntem seçilmelidir. Kliniğimizde spinal aneste- zi (SA) ve genel anestezi (GA) uygulanmakta olup, her iki yöntemin maternal etkilerini geriye dönük değerlendirmeyi amaçladık. Gereç ve Yöntemler: Kliniğimizde 2012-2013 yılları ara- sında gerçekleştirilen sezaryen operasyonlarından 50 spi- nal, 50 genel anestezi uygulanmış, cerrahi operasyon ve sistemik hastalığı bulunmayan 100 hasta çalışma kapsa- mına alındı. Hastaların demografik özellikleri, cerrahinin toplam süresi, fetusun doğumuna kadar geçen süre, ope- rasyon öncesi ve sonrası hemogram değerleri, hastane- de yatılan gün sayısı ve ile postoperatif dönemde gelişen komplikasyonlar kaydedildi. Bulgular: Genel anestezi ve spinal anestezi uygulanan olgular arasında yaş, gravida, parite, fetusun doğumuna kadar geçen süre, hastanede yatılan gün sayısı ve kompli- kasyon oranı bakımından istatistiksel olarak anlamlı fark gözlenmemiştir. Olguların %20sinde postoperatif dönemde çeşitli komplikasyonlar gelişmiştir. Genel ve spinal anestezi uygulanan hastalar arasında komplikasyon oranı açısından anlamlı fark yoktur. Genel anestezi uygulanan olguların operasyon süreleri, spinal anestezi uygulanan olgulara göre istatistiksel olarak anlamlı düzeyde yüksektir. Operasyon öncesi ve sonrası maternal hemoglobin ve hematokrit dü- şüşü, genel anestezi yapılan gebelerde, spinal anestezi yapı- lanlara göre istatistiksel olarak anlamlı düzeyde yüksektir. Sonuç: Sezaryen operasyonlarında spinal ve genel anes- tezinin fetusun doğumuna kadar geçen süre, hastanede yatılan gün sayısı, postoperatif komplikasyonlar açısından birbirlerine üstünlükleri yoktur. Ancak spinal anestezi kul- lanımı daha az kan kaybı ve daha kısa operasyon süreleri sağlamıştır. Özellikle maternal anemi ve uterin atoni ris- kinin daha fazla olduğu durumlarda spinal anestezi tercih edilmelidir.

Abstract

Objective: The choice of anesthesia method in cesarean section is important. The most appropriate method for the maintenance of the health of the mother and the fetus must be selected. In our clinic, both spinal anesthesia (SA) and general anesthesia (GA) have been applied and in this pa- per we aimed to evaluate the maternal effects of the two methods retrospectively. Material and Methods: In this study, 100 patients who un- derwent cesarean delivery between 2012-2013, ( 50 spinal, 50 general anesthesia,) had no surgery and systemic dise- ase were included. Demographic data, duration of surgery, time to the birth of the fetus, preoperative and postoperative complete blood count, duration of hospitalization and pos- toperative complications were recorded. Results: Among patients who underwent general anesthe- sia and spinal anesthesia, there was not any statistically significant difference in terms of age, gravida, parity, time to expulsion of the fetus, the duration of hospitalization and complication rate. Various complications occurred in 20% of patients in the postoperative period. Complication rate was not significantly different between patients who under- went general anesthesia and spinal anesthesia. Duration of surgery in patients who underwent general anesthesia was statistically significantly higher than in other patients. Pre- operative and postoperative decrease in maternal hemog- lobin and hematocrit levels were statistically significantly higher in general anesthesia than in spinal anesthesia. Conclusion: General and spinal anesthesia in cesare- an section have no superiority to each other, in terms of time to expulsion of the fetus, the number of hospitalized days and postoperative complications. However, the use of spinal anesthesia gives rise to less blood loss and shorter operative times. Spinal anesthesia should be preferred, es- pecially in cases where maternal anemia and uterine atony risk is high.
