Megadoz Steroid ile İndirekt Travmatik Optik Nöropati Tedavisi

Özet

Amaç: Travmatik optik nöropati (TON) nadir görülen bir durum olmasına rağmen, görme kayıplarının önemli bir nedenidir. Cerrahi dekompresyon, sadece izlem ve farklı dozların kullanıldığı steroid dozlarının kullanıldığı değişik tedavi yaklaşımları önerilmesine rağmen, bu yıkıcı görme kaybı nedeni olan durumun tedavisinde standart bir yaklaşım geliştirecek kesin kanıtlar saptanamamıştır. Bu hastalarda kullanılan intravenöz mega doz steroid tedavisinin etkinliği incelendi. Gereç ve Yöntem: Ekim 2009-Mayıs 2013 arasında indirekt TON nedeniyle yüksek doz kortikosteroid tedavisi uygulanan 16 hastanın dosyaları gözden geçirildi. Ortalama yaşları 29 (16-56 yıl) olan 16 hasta (12 erkek, 4 bayan) çalışmaya dahil edildi. Tüm olgular 30mg/kg metil prednizolon yükleme dozu ve devamında 48 saat sürede verilen 5,4 mg/kg/saat idame dozu aldılar. Bulgular: Tedaviden sonra, 16 gözden 10’unda (%62) görme keskinliği arttı, 6 gözde (%38) ise aynı kaldı. Travma ile tedavinin başlangıcı arasındaki ortanca süre, görme artışı olan hasta grubunda 4,6 gün (1-11 gün), görmenin aynı kaldığı grupta ise 10,5 gündü. H içbir hastada tedaviye bağlı ciddi bir komplikasyon gelişmedi. Sonuç: Nöroftalmolojik bir acil olan travmatik optik nöropatiye erken müdahale önemlidir. Bazı hastalarda, erken başlangıçlı intravenö z megadoz steroid tedavisi görme keskinliğinin korunmasında faydalı olabilmektedir.

Abstract

bjective: Although uncommon, traumatic optic neuropathy (TON) is an important cause of visual loss. Different therapeutic approaches including different dosages of steroids, surgical decompression of optic canal and observation alone have been suggested but th ere has been no conclusive evidence to establish a Standard approach to this devastating cause of visual loss. The effectiveness of intravenous (IV) mega-dose steroids in treatment of these patients were reviewed. Material and Method: The medical records of 16 patients who received mega dose corticosteroid treatment because of indirect TON, be- tween October 2009 and May 2013, were reviewed. Sixteen patients (12 males, 4 females) with mean age 29 (16 to 56 years) were enrolled. All patients had received 30 mg/kg loading dose of methylprednisolone followed by 5.4 mg/kg/hour for 48 hours. Results: After treatment, the visual acuity improved in 10 of 16 eyes (62%) and unchanged in 6 eyes (32%). The light perception was neg- ative at the time of diagnosis in 2 of the 5 eyes without visual improvment. The median time between the trauma and initiation of the treatment was 4.6 days (range 1-11 days) in group of patients whose vision improved and 10.5 days in the group of patientswhose vision did not change. No serious complication due to treatment developed in any patient. Conclusion: Is a neuro-ophthalmic emergency and early intervention is important. Early initiation of Intravenous megadose methylpred- nisolone treatment, can save useful vision in some cases.
