Büyüme Hormonu Tedavisi Alan Çocuklarda Ön Segment Parametreleri ve Aksiyel Uzunluğun Değerlendirilmesi

Özet
Amaç: Konjenital izole büyüme hormonu eksikliği nedeniyle büyüme hormonu tedavisi alan hastaların topografik kornea ve ön segment parametrelerini değerlendirmek, bu bulguları sağlıklı gönüllüler ile karşılaştırmak ve büyüme hormonu tedavisinin gözün ön segment parametreleri üzerinde herhangi bir etkisi olup olmadığını değerlendirmek. Gereç ve Yöntem: Bu karşılaştırmalı çalışmaya konjenital izole büyüme hormonu eksikliği olan 30 hastanın 30 gözü ve 30 sağlıklı katılımcı dahil edildi. Tüm katılımcılara tam oftalmolojik muayene yapıldı. Korneal topografik ölçümler ve ön segment biyometrisi Scheimpflug topografi cihazı kullanılarak değerlendirildi. Aksiyel uzunluk, temassız kısmi koherans interferometre ile ölçüldü. Bulgular: İki grup arasında yaş ve cinsiyet açısından anlamlı fark yoktu (p=0,174 ve p=0,174). Büyüme hormonu tedavisi alan çocuklarda ortalama merkezi kornea kalınlığı ve kornea hacmi sağlıklı bireylere göre istatistiksel olarak anlamlı derecede yüksek bulundu (p=0,042 ve p=0,01). Ancak iki grup arasında keratometri değerleri, ön kamara derinliği, ön kamara hacmi, iridokorneal açı genişliği, aksiyel uzunluk ve kırma kusurları açısından istatistiksel olarak anlamlı fark bulunmadı (p>0,05). Sonuç: Merkezi kornea kalınlığı ve kornea hacmi değerleri büyüme hormonu tedavisi alan hastalarda daha yüksekti. Bulgularımız, gözün büyüme hormonu için hedef organlardan biri gibi göründüğünü göstermektedir. Farklı oküler dokularda büyüme hormonunun tam mekanizmasını tanımlamak için tanımlayıcı çalışmalara ihtiyaç vardır.
Abstract
Objective: To evaluate the topographic corneal and anterior segment parameters of patients who had growth hormone therapy due to congenital isolated growth hormone deficiency, to compare these findings with those of healthy subjects and to find out whether growth hormone therapy has any effect on anterior segment parameters of the eye. Material and Method: Thirty eyes of 30 patients with congenital isolated growth hormone deficiency and 30 healthy participants were included in this comparative study. All subjects underwent complete ophthalmologic examination. Corneal topographic measurements and anterior segment biometry were assessed using the Scheimpflug topography device. Axial length was measured with a noncontact partial coherence interferometer. Results: There were no significant differences between the two groups in terms of age and gender (p=0.174 and p=0.174). The mean central corneal thickness and corneal volume in children receiving growth hormone therapy were found to be statistically significantly higher than in healthy subjects (p=0.042 and p=0.01). However, no significant difference was found between the the groups in terms of keratometry values, anterior chamber depth, anterior chamber volüme, iridocorneal angle width, axial length, and refractive errors (p>0.05). Conclusion: Central corneal thickness and corneal volume values were higher in patients receiving growth hormone therapy. Our findings suggest that the eye seems to be one of the target organs for growth hormone. Descriptive studies are needed to define the exact mechanism of growth hormone on different types of ocular tissues.
