Anaesthesia Preferences for Cataract Surgery: A Survey Study

Özet

Amaç: Fakoemülsifikasyon yöntemi ile katarakt cerrahisi, tüm dünyada en fazla uygulanan cerrahi işlemlerin başında gelmektedir. Bu anket çalışması ile amacımız ankete katılan göz hekimlerinden ülkemizde bu cerrahi için tercih edilen anestezi yöntem ve tekniklerini araştırmak, dünyadaki uygulamalarla kıyaslama yapmak. Gereç ve Yöntem: Hekimlerin demografik özelliklerini ve fakoemülsifikasyon yöntemi ile katarakt cerrahisi için kullandıkları anestezi yöntem ve tekniklerini araştıran standardize anket formları, ülkemizdeki çeşitli hastanelerde görev yapan oftalmologlara elektronik ortamda ulaştırıldı. Bulgular: Anketi tamamlayan 162 hekimin cevapları incelendiğinde, fakoemülsifikasyon yöntemi ile katarakt cerrahisi için %80,3 oranında lokal anestezi tercih edildiği görüldü. Lokal anestezi uygulaması için genellikle topikal (%33,3) veya topikal + intrakamaral anestezi tercih edildiği (%38,3), topikal uygulama dışındaki lokal anestezi uygulamaları için en sık kullanılan anestezik maddenin ise lidokain olduğu görüldü (%75,3). Hekimlerin yanıtları, lokal anestezi ile yapılan vakaların %99’unda anestezi uygulamasının cerrah tarafından yapıldığını ve %53’ünde cerrahi sırasında ameliyat salonunda göz hekimleri dışında anestezi eğitimi almış bir personelin bulunmadığını göstermiştir. Hekimlerin cerrahi için lokal anestezi yerine genel anestezi tercih etmesinin en sık sebebinin cerrahi nedeniyle anksiyete yaşayan hastalar olduğu görülmüştür (%72,8). Sonuç: Çalışmamızın sonuçları, fakoemülsifikasyon yöntemi ile katarakt cerrahisi için ankete katılan göz hekimlerinin tercih edilen anestezi yöntem ve tekniklerinin dünyadaki uygulamalarla genel olarak benzer olduğunu fakat lokal anestezi uygulamaları için dünyadaki diğer ülkelere kıyasla anestezistlerin çok az görev aldığını göstermiştir.

Abstract

Objective: Cataract surgery with phacoemulsification technique is one of the most common surgical procedures in the world. Our aim with this survey study is to research the anesthesia methods and techniques preferred for this surgery in our country by the ophthalmologists who participated in the survey, and to make comparisons with the applications in the world. Material and Method: Standardized survey forms, which investigate the demographic characteristics of physicians and the anesthesia methods and techniques they use for cataract surgery with phacoemulsification technique were delivered to ophthalmologists who work in various hospitals in our country. Results: Responses of the 162 physicians who completed the survey were evaluated. Results of the survey showed that local anesthesia was preferred at a rate of 80.3% for cataract surgery with phacoemulsification technique. Topical (33.3%) or topical + intracameral anesthesia (38.3%) was generally preferred for local anesthesia; and lidocaine was the most commonly used anesthetic agent for local anesthesia other than topical anasthesia (75.3%). The responses of the surgeons showed that in %99 of cases local anesthesia (38.3%) administrations were performed by surgeons and in 53% of the cases, there was no anesthesia trained person except ophthalmologists in the room during surgery. It was observed that the most common reason for surgeons to prefer general anesthesia instead of local anesthesia for cataract surgery was patients anxiety (72.8%). Conclusion: The results of our study has revealed that the anesthesia methods and techniques preferred by the 162 ophthalmologist who responded to the survey for cataract surgery with phacoemulsification technique are similar to most other countries. However, our results also has shown that anaesthesiologists have a little role for local anesthesia applications compared to other countries in the world.
