Neovasküler Tip Yaşa Bağlı Maküla Dejeneresansında Vitreus İçine Anti-Vasküler Endotelyal Büyüme Faktörü Uygulamalarının Koroid Kalınlığına Etkisi

Özet

Amaç: Neovasküler tip yaşa bağlı maküla dejeneresansı olgularında, vitreus içine anti-vasküler endotelyal büyüme faktörü uygulamaları ile subfoveal koroid kalınlığındaki değişimi değerlendirmek. Gereç ve Yöntem: Bu kesitsel çalışmada neovasküler tip yaşa bağlı makula dejeneresansı nedeniyle vitreus içine anti-vasküler endotelyal büyüme faktörü uygulaması yapılmış 46 olgunun 50 gözü ile 25 kontrol olgusunun 50 gözü karşılaştırıldı. Spektral domain optik koherans tomografi ile santral maküla ve subfoveal koroid kalınlıkları değerlendirildi. Olgular, vitreus içine anti-vasküler endotelyal büyüme faktörü tedavi sayısına göre gruplara ayrıldı. Grup 1; 1-5 uygulama yapılan, Grup 2; 6’dan fazla uygulanmış olgular olarak sınıflandırıldı. Bulgular: Grup 1’de 28 göz ve grup 2’de 22 göz mevcuttu. Olguların 24’ü (%52) kadın, 22’si (%48) erkekti. Yaş ortalaması 72,5±8,3 yıl (53-89 yıl) idi. Daha fazla uygulama yapılan grupta santral maküla kalınlığı anlamlı olarak ince bulunurken, en iyi düzeltilmiş görme keskinliği ve subfoveal koroid kalınlıkları açısından istatistiksel olarak anlamlı fark saptanmadı. Sonuç: Neovasküler tip yaşa bağlı maküla dejeneresansı olgularında farklı anti-vasküler endotelyal büyüme faktörü uygulanma sayıları ile subfoveal koroid kalınlığında değişim olmamaktadır. Vitreus içine anti-vasküler endotelyal büyüme faktörü tedavisi farmakolojik etkisini neovasküler dokuda göstermekte ve koroid dolaşımını etkilememektedir.

Abstract

Objective: We aimed to evaluate the changes in subfoveal choroidal thickness in cases of exudative age-related macular degeneration with intravitreal anti-vascular endothelial growth factor therapy. Material and Method: In this cross-sectional study, patients who received anti-vascular endothelial growth factor injections due to neovascular type age-related macular degeneration were included. The cases were divided into groups according to the number of intravitreal anti-vascular endothelial growth factor injections. The cases were classified as; Group 1: 1-5 injections and Group 2: cases with more than 6 injections. The control group consisted of age- and gender-matched cases who did not receive intravitreal anti-vascular endothelial growth factor injections. The central macula and subfoveal choroidal thicknesses were compared using spectral domain optical coherence tomography. Results: A total of 50 eyes of 46 patients in the treatment group and 50 eyes of 25 patients in the control group were enrolled. There were 28 eyes in group 1 and 22 eyes in group 2. Twenty-four (52%) of the treated patients were female, 22 (48%) were male, and the mean age was 72.5±8.3 (53-89) years. No significant difference was found between groups 1 and 2 in terms of best corrected visual acuities (p=0.06). Central macular thickness was found to be significantly thinner in the group with more injections (Group 2) than in the patients with less injections (Group 1) (p=0.02). However, no statistically significant difference was found between the groups regarding subfoveal choroidal thickness (p=0.416). Conclusion: Central macular thickness decreases with anti-vascular endothelial growth factor injections in cases of exudative age-related macular degeneration, but no change was detected in subfoveal choroidal thickness. Intravitreal anti-vascular endothelial growth factor therapy shows its pharmacological effect on neovascular tissue and does not affect the choroidal circulation.
