Özet
Seksen yedi yaşında kadın hasta, topikal tedavi altında 10 gündür süren endoftalmi tanısıyla kliniğimize refere edildi. Görme keskinliği sağ gözde 0,8, solda el hareketiydi. Sağ göz tüm muayene bulguları normaldi. Sol biyomikroskopik muayenesinde konjonktival hiperemi, kemozis, korneal ödem ve hipopyon izlendi. Ayrıca periorbital ödem, hiperemi, üst göz kapağında temporalde belirgin şişlik, bu bölgeden kaynaklanan pürülan akıntı ve göz hareketlerinde kısıtlılık görüldü. Sol fundus seçilemedi. Radyolojik görüntülemesi endoftalmiye sekonder orbital apse olarak yorumlanan hastanın kültürleri ve kanları alınıp yatışı yapıldı. Sistemik ve topikal antibiyoterapi başlandı. Takiplerinde temporaldeki apse formasyonundan spontan pürülan materyal drenajı izlendi. İki haftalık antibiyoterapi sonrası kliniğinde anlamlı düzelme gözlendi. Endoftalmi, sekonder orbital apse sebeblerinden biri olabilir. Dirençli ve tedavide geç kalınmış endoftalmi vakalarında endoftalmiye sekonder orbital apse düşünülmelidir.
Abstract
Eighty seven year old female patient was referred to our clinic with the diagnosis of endophthalmitis lasting 10 days under topical treatment. Visual acuity was 0.8 in the right eye and hand motion in the left. All examination findings in the right eye were normal. Left eye slit-lamp examination revealed conjunctival hyperemia, chemosis, corneal edema, and hypopyon. In addition, periorbital edema, hyperemia, significant temporal swelling of the upper eyelid, purulent discharge originating from this area and limitation of eye movements were observed. The left fundus could not be evaluated. The radiological imaging of the patient was interpreted as orbital abscess secondary to endophthalmitis. The cultures and blood of the patient were taken and the patient hospitalized. Systemic and topical antibiotherapy was started. In the follow-up, spontaneous drainage of purulent material from the abscess formation in the temporal was observed. After two weeks of antibiotherapy, a significant improvement was observed in his clinic findings. Endophthalmitis may be one of the causes of secondary orbital abscess. Orbital abscess secondary to endophthalmitis should be considered in cases of resistant endophthalmitis.
