Hipertansiyon için Renin Anjiyotensin Aldosteron Sistemi İnhibitörleri Kullanan Hastalarda COVID-19: Akciğer Tutulumu, Yoğun Bakım Ünitesine Kabul ve Mortalite ile İlişkili Faktörlerin Değerlendirilmesi

Özet

Amaç: Renin anjiyotensin aldosteron sistemi inhibitörü kullanımı ile toraks bilgisayarlı tomografide akciğer tutulumu, yoğun bakım üni‐ tesine kabul ve mortalite gibi koronavirüs hastalığı‐2019 (COVID‐19) ile ilgili özellikler arasındaki olası ilişkileri araştırmak.. Gereç ve Yöntem: COVID‐19 enfeksiyonu tanısı ile başvuran ve eş zamanlı toraks bilgisayarlı tomografi taraması yapılan toplam 654 hastadan 220 hasta çalışmaya dahil edildi. Hastalar hipertansiyon olmayan grup, renin anjiyotensin aldosteron sistemi grubu ve kalsi‐ yum kanal blokeri grubu olarak üçe ayrıldı. Bulgular: 220 hastanın 117'si (%53,2) erkekti ve gruplar arasında cinsiyet açısından fark görülmedi. Hastane içi mortalite, 1 aylık mor‐ talite, mekanik ventilasyon ihtiyacı, akciğer tutulumu ve diğer radyolojik bulgular açısından gruplar arasında anlamlı fark gözlenmedi. Albümin, C‐reaktif protein düzeyi, beyaz küre, trombosit sayısı ve erkek cinsiyeti hastanede yatan hastalar arasında yoğun bakım ünite‐ sine kabulde bağımsız bir öngördürücü olarak izlend. Başka bir sonuç, yaşın ve serviste kalış süresinin yoğun bakım ünitesine yatan has‐ talarda ölümün bağımsız bir öngördürücüsü olduğu saptandı. Sonuç: Çalışmamızda hipertansiyon için renin anjiyotensin aldosteron sistemi inhibitörleri kullanan, kalsiyum kanal blokeri kullanan ve hipertansiyon veya diğer kronik hastalık öyküsü olmayan hastaların toraks bilgisayarlı tomografide akciğer tutulumu açısından farklılık olmadığı gösterildi. Ayrıca hipertansiyonun yoğun bakım ünitesine yatış ve ölümle ilişkili olmadığı da bulundu. Albümin, C‐reaktif prote‐ in düzeyi, trombosit, beyaz küre sayısı ve erkek cinsiyet, yoğun bakım ünitesine kabulün bağımsız belirleyicileridir ayrıca, yoğun bakım ünitesinde yatan hastalarda yaş ve serviste kalış günleri ölümün bağımsız bir öngördürücüsüdür.

Abstract

Objective: To investigated possible relationships between renin angiotensin aldosterone system inhibitor use and coronavirus disease 2019 (COVID‐19) related characteristics such as lung involvement on computed tomography, admission to intensive care unit, and mortality. Material and Method: A total of 220 patients out of 654 patients who applied with the diagnosis of COVID‐19 infection and had simultaneous thorax computed tomography scan were included in the study. The patients were divided into three groups: the non‐hypertension group, the renin‐angiotensin aldosterone system group, and the calcium channel blocker group. Results: Of the 220 patients, 117 (53,2%) were males and no difference was observed between the groups in term of sex. No significant diffe‐ rence was observed between the groups in terms of in‐hospital mortality, 1‐month mortality, mechanical ventilation need, lung involvement and other radiological findings. Albumin level, C‐reactive protein level, white blood cell count, platelet count and male gender were determined as factors that were independently associated with intensive care unit admission among hospitalized patients. Also, age and length of stay in ward were found to be independently associated with mortality in patients admitted to the intensive care unit. Conclusion: In our study, we showed that patients using renin angiotensin aldosterone system inhibitors for hypertension, those using calcium channel blocker, and individuals without a history of hypertension or other chronic diseases did not differ in terms of lung involvement on com‐ puted tomography. We also found that hypertension presence was not associated with intensive care unit admission and death. Albumin level, C‐reactive protein level, platelet count, white blood cell count and male gender are independent predictors of intensive care unit admission; also found that age and length of ward stay days were independent predictors of death in patients hospitalized in the intensive care unit.
