Gastroözofagial varislerin büyüklüğü ile Child Pugh skoru arasında bir ilişki var mı?

Özet

Bu çalışmada, sirotik hastalardaki varis büyüklüğü ile hastaların Child Pugh skoru arasındaki ilişkinin araştırılması amaçlandı. Gastroenteroloji polikliniğimizde takipli olan 186 varisli sirotik hasta retrospektif olarak değerlendirildi. Child Pugh skorlamasına göre hastalar Chid A sınıfı (5-6 puan), Child B sınıfı (7-9 puan) ve Child C sınıfı (10-15 puan) olarak gruplandırıldı. Endoskopik olarak saptanan özofagial ve gastrik varisler büyüklüklerine göre küçük (10 mm) olarak tanımlandı. Hastaların 154’ü (%82,7) özofagus varisli, 32’si (%17,3) gastrik varislilerden oluşmaktaydı. Gastrik varislilerin 28’i (%87,5) gastroözofagial varisli hastalardan 4’ü (%12,5) ise izole gastrik varisli hastalardan oluşmaktaydı. Özofagus varislilerde Child Pugh sınıflarına göre gruplar arasında varis büyüklükleri açısından istatistiksel olarak anlamlı bir farklılık saptanmadı (P>0,05). Gastrik varislilerde de Child Pugh sınıflarına göre gruplar arasında varis büyüklükleri açısından istatistiksel olarak anlamlı bir farklılık saptanmadı (P>0,05). Sonuç olarak, çalışmamızda sirotik özofagus varisli ve sirotik gastrik varisli hastalarda Child Pugh Skoru ile varis büyüklüğü arasında anlamlı bir ilişki saptanmadı.

Abstract

In this study, our objective was to investigate the relationshipbetween the size of gastroesophageal varices and Child Pughscores in cirrhotic patients. Retrospectively designed study included 186 cirrhotic patients with gastroesophageal varices whohad been followed up by our gastroenterology polyclinic. The patients were classified into three groups according to their ChildPugh scores as Child A (5-6 points), Child B (7-9 points), Child C(10-15 points). Endoscopically detected gastric and esophagealvarices were classified according to their sizes as small (10 mm) varices. In our study82.7% (n=154) of the patients had esophageal varices and 17.3%(n=32). gastric varices Gastroesophageal varices were found in%87.5 (n:28) and isolated gastric varices in 12.5% (n:4) of the patients with gastric varices. There were no statistically significantdifference in sizes of the esophageal varices between the ChildPugh scoring groups (p>0.05). Furthermore, we did not find anystatistically significant intergroup differences between size of thegastric varices and Child Pugh scores (p>0.05). As a conclusion, incirrhotic patients with gastric or esophageal varices, we did notfind any significant correlation between the size of the varicesand the Child Pugh scores of the patients.Keywords: Child Pugh score, gastric varices,
