Özet

Amaç: Saddle nose deformitesi, septumun travma, ameliyat, inflamatuar hastalıklar, tümörler ve intranazal ilaç kullanımı gibisebeplerden oluşabilen bir hastalıktır. Saddle nose deformitesinin tedavisinde beklenen eksik olan dokuyu tekrar oluşturmak ve burnunsolunum fizyolojisini geri kazandırmaktır. Bizim yeni septum modellememizde ki amaç hem doğal septum anatomisine en yakın septumuelde etmek hem de yeni septum modelinin sonuçlarını değerlendirmektir.Gereç ve yöntemler: Çalışmaya ileri saddle nose deformitesi nedeniyle palisad tekniği uygulanan 9 hasta dahil edilmiştir. Hastaların 3’ükadın 6’sı erkek ve yaş ortalamaları 35,2 (26-48) yıldı. Hastaların 3 tanesi daha önce rinoplasti operasyonu geçirmiş, 5 hasta çocuklukçağında nazal travma öyküsü mevcut, 1 hastanın ise anamnezinde sebebe yönelik bilgiye ulaşılamadı. Hastalara acık rinoplasti yöntemiyleyaklaşıldı. Bu çalışmada septumu oluşturmak için kostal kartilaj kullanıldı ve kostal kartilajdan hazırlanan şeritler palisad şeklinde hazırlanıpyeni septum oluşturuldu. Yeni oluşturulmuş septumun sırt bölgesine spreader greft şeklinde iki adet kartilajla desteklendi. Palisad şeklindeoluşturulan septum yeni yerine yerleştirilip tabanda krest bölgesindeki mukozaya, önde alar kartilaj medial kruslarına ve arkada perpedikularbölgeye sütürlerle sabitlendi. Hastaların 1 yıl sonrası sonuçları değerlendirildi.Bulgular: Hastalar pos-op birinci yılında yapılan kontrollerinde fonksiyonel ve kozmetik sorunlarla karşılaşılmadı. Kontrol süresiboyunca revizyon cerrahisine ihtiyaç duyulacak burunda eğrilme ve çökme şikayetleri gözlemlenmedi. Hastaların anamnezlerine dayanaraknefes almalarının ameliyat öncesine göre daha iyi olduğunu bildirdiler.Sonuç: Ağır saddle nose deformitelerinde, septumu oluşturmak için bu yeni yöntem güvenle uygulanabilir.

Abstract

Objective: Saddle nose deformity is a disease that can be caused by septum trauma, surgery, inflammatory diseases, tumors andintranasal drug use. The aim of the treatment is to regenerate the missing tissue and restore the respiratory physiology of the nose. Thepurpose of our new septum modeling is to obtain the closest septum to the natural septum anatomy and to evaluate the results of the newseptum model.Material and Methods: 9 patients who underwent palisade technique due to advanced saddle nose deformity were included in the study.3 of the patients were female and 6 were male and their average age was 35.2 (26-48) years. 3 of the patients had a previous rhinoplastyoperation, 5 patients had a history of nasal trauma in childhood, and 1 patient had no information about the cause in the anamnesis. Patientswere approached using the open rhinoplasty method. In this study, a costal cartilage was used to create the septum, and a new septum wascreated by preparing the strips prepared from the costal cartilage in the form of a palisade. The dorsal area of the newly formed septum wassupported with two cartilages in the form of spreader grafts. The Palisade-shaped septum was placed in its new place and fixed with suturesto the mucosa in the crest region at the base, the medial cruses of the alar cartilage at the front and the perpedikular area at the back. Theresults of the patients after 1 year were evaluated.Results: A year after the operation, there were no functional and cosmetic problems. During the control period, there were nocomplaints of bending and collapsing of the nose that would require revision surgery. Based on the anamnesis of the patients, they reportedthat their breathing was better than before the surgery.Conclusion: This new method can be safely applied to create the septum in severe saddle nose deformities.
