COVID 19 SONRASI UZAMIŞ KOKU BOZUKLUĞU OLAN HASTALARIN KOKU TERAPİSİ İLE DEĞERLENDİRİLMESİ

Özet
Amaç: Long-COVID"de koku ve tat alma disfonksiyonu sık görülen semptomlardır. Bu çalışmada long-COVİD semptomu olarak uzamış koku ve tat alma bozukluğu semptomu olan hastaları takip ettik ve koku terapisinin etkinliğini araştırmayı amaçladık. Gereç ve Yöntemler: COVİD 19 enfeksiyonu sonrası 1 aydan uzun süren koku bozukluğu şikayeti ile başvuran hastalar çalışmaya dahil edildi. Hastalar koku ve tat duyusunu 0'dan 10'a kadar bir görsel analog skala (VAS) derecelendirildi. Hastalara gül, okaliptüs, limon, karanfil uçucu yağlarını içeren koku terapisi önerildi. 3 ay sonra kontrole çağrılarak koku terapisine uyumları sorgulandı ve koku ve tat kaybı tipi sorgulandı ve tekrar VAS aracılığıyla derecelendirildi. Bulgular: Çalışmaya %63.6"sı (n= 21) kadın ve %36.4"ü (n=12) erkek 33 hasta dahil edildi. Koku terapisini (KT) 23 (%69.7) hasta ortalama 4.6±4.2 hafta uyguladı. 10 (%30.3) hasta hiç KT yapmamış,13 (%39.4) hasta 8 haftadan daha az ve 10 (%30.3) hasta da 8 hafta ve daha fazla süreyle uygulamıştır. KT uygulayan ve uygulamayan grupta tedavi öncesinde ve tedavi sonrasında koku VAS skorları (p= 0524 ,p=0.832) tat VAS skorları benzerdi (p= 0.630 , p=0.576). COVİD 19 enfeksiyonu sonrası birinci ayda 26 (%78.8) hasta anosmi, 3(%9.1) hasta hiposmi, 4 (%12.1) hasta parosmi bildirmiş iken; üçüncü ayın sonunda 2(%6.1) hasta anosmi, 8(%24.2) hasta hiposmi, 12(%36.4) hasta parosmi bildirmiş, 11(%33.3) hasta da düzeldiğini ifade etmiştir. Sonuç: Çalışmamızda, koku terapisi uygulayan hastalarda koku alma fonksiyonunda anlamlı bir artış gözlemlemedik. Bununla birlikte, COVID-19 hastalarında koku alma ve tat alma bozuklukları ile ilişkili semptomların zamanını ve kalıcılığını analiz eden ve tedavi yöntemlerini araştıran daha geniş çalışmalara ihtiyaç vardır.
Abstract
Objective: Dysfunction of smell and taste are common symptoms in Long-COVID. In this study, we followed patients with smell and taste disorders as a long-COVID symptom and aimed to investigate the effectiveness of olfactory training. Methods: Patients with olfactory disorder lasting longer than 1 month after the COVID 19 infection were included in the study. Patients reported their sense of smell and taste on a visual analog scale (VAS) from 0 to 10. Olfactory training(OT) including essential oils of rose, eucalyptus, lemon and clove was recommended to the patients. After 3 months, they were called for control and their compliance with OT was questioned and their loss of smell and taste was questioned by VAS. Results: 33 patients, 63.6% (n= 21) female and 36.4% (n=12) male, were included in the study. 10 (30.3%) patients did not perform any OT, 13 (39.4%) patients applied it for less than 8 weeks and 10 (30.3%) patients for 8 weeks or more. The olfactory VAS scores (p= 0.524 ,p=0.832) and taste VAS scores were similar (p= 0.630 , p=0.576) in the groups. In the first month, 26 (78.8%) patients reported anosmia, 3 (9.1%) hyposmia, and 4 (12.1%) parosmia; At the end of the third month, 2 (6.1%) patients reported anosmia, 8 (24.2%) hyposmia, 12 (36.4%) parosmia, and 11 (33.3%) patients recovered. Conclusion: In our study, we did not observe a significant increase in olfactory function in patients who applied OT. However, larger studies analyzing olfactory and taste disorders in COVID-19 patients are needed.
