Özet
kniğinin temel amacı, perforasyon alanını kapatan, kronik otitis media sekellerini önleyen ve işitsel iyileşme sağlayan greft materyali ile sağlam, kuru bir kulak zarı oluşturmaktır. Bu çalışmada amacımız, kronik otitis media nedeniyle timpan membranda geniş perforasyonu olan ve endoskopik transkanal kondro-perikondriyal greft kullanılarak, timpanoplasti ameliyatı olan hastalarımızı değerlendirmeye almaktır. Gereç ve Yöntemler: Çalışmaya 1 Ocak 2019- 1 Ocak 2021 tarihlerinde endoskopik timpanoplasti yapılan 38 hasta retrospektif olarak dahil edildi. Çalışmaya >5 mm santral perforasyonu olan, preoperatif saf ses odyometrisi yapılmış ve en az 3 ay kulak akıntısı şikayeti olmayan endoskopik tip1 timpanoplasti ameliyatı yapılan hastalar dahil edildi. Bulgular: Endoskopik timpanoplasti cerrahisi yapılan 38 hastanın yaş ortalaması 23,55 ± 10,05 yıl ve kadın-erkek oranı 21:17 olarak hesaplandı. Ortalama ameliyat süresi 44,05 ± 5,57 dakika olarak saptandı. Hastaların 35'inde ameliyat sonrası altıncı ayda greftin intakt olduğu saptandı (%92,1). Postoperatif 6. ayda yapılan işitme testi ölçümlerinde ameliyat sonrası hava-kemik aralığı değerleri, ameliyat öncesi testlere göre istatistiksel anlamda olumlu düzelme izlendi. (p
Abstract
Objective: Tympanoplasty is a surgical technique that is used in chronic otitis media surgery. The main purpose of the endoscopic tympanoplasty is to create a solid, dry eardrum with graft material that closes the perforation area, prevents chronic otitis media sequelae and provides auditory recovery. In this study, our aim is to evaluate our patients who had tympanoplasty using endoscopic transcanal chondroperichondrial graft and who had large perforation of the tympanic membrane due to chronic otitis media. Material and Methods: 38 patients who had surgery, endoscopic tympanoplasty January 1, 2019 -January 1, 2021 were retrospectively included in the study. Patients with >5 mm central perforation, had preoperative pure tone audiometry, and no ear disease for at least 3 months, who had endoscopic type 1 tympanoplasty surgery were included in the study. Results: The mean age of 38 patients who had endoscopic tympanoplasty was 23.55 ± 10.05 years and the female-male ratio was calculated as 21:17. The mean operation time was 44.05 ± 5.57 minutes. In 35 of the patients, the graft was intact at the 6th month postoperatively (92.1%). In the postoperative 6th month hearing test measurements, postoperative air-bone gap values and statistically positive improvement were observed compared to the preoperative tests.(p
