Topikal göz ilaçlarının dikkatsiz kullanımı sonucu oluşan mekanik oküler yüzey yaralanmaları

Özet
Amaç:Göz damlaları veya merhem şişe uçlarının konjonk tiva ve korneaya teması ile oluşan kızarık ve ağrılı gözlerin kliniği ve tanısı değerlendirildi. Gereç ve Yöntem: Göz damlaları veya göz merhemleri kullanırken ani olarak ortaya çıkan ağrılı ve kızarık gözlerle başvuran hastaların kayıtları geriye dönük olarak tarandı. Biyomikroskopik muayenede konjonktiva veya korneasında damla uçlarının tekrarlayan temasına bağlı gelişen erozyonlu has talar çalışmaya dahil edildi. Tüm hastalara ilaçlarını nasıl kul landıkları soruldu ve hepsinde ilaç şişe uçlarının konjonktiva ve/veya korneaya teması gözlemlendi. Bulgular:Toplam beş hastanın altı gözünde yanlış ilaç uy gulanması sonucu konjonktiva veya korneada erozyon gö rüldü. Beş gözde erozyon alt bulbar konjonktivada harita şeklinde, bir hastada ise sağ göz saat yedi kadranında kor nea ve konjonktiavada oluşmuştu. Son hastanın görmesi ışık seviyesinde idi ve antiglokomatöz ilaçlar kullanıyordu. Bir hasta katarakt cerrahisi sonrası göz damlaları, üç hasta ise oftalmik pomatlar kullanıyordu. Tüm hastaların konjon tivasında boyanma, aynı bölgede lokalize hiperemi ve bir hastada kornea erozyonuna ikincil olarak enfeksiyöz keratit gelişimi izlenildi. Konjonktiva erozyonlarında tedavi, suni gözyaşı damlaları ve kapama ile yapıldı. Sonuç:Hastalara ilaçları gözden daha uzak mesafeden kul lanmaları anlatılmalıdır. Özellikle az gören hastalara göz ilaç larının başkaları tarafından damlatılması önerilmelidir. Göz ilaçları kullanırken ani ortaya çıkan lokalize kızarıklıklar ve ağrılarda hastalar tarafından yanlış ilaç kullanımı akla gelmelidir.
Abstract
Background: We aimed to report the clinical features and diagnosis of red and painful eyes caused by contact between the tip of an eye drop or ointment bottle and the conjunctiva or cornea. Methods:The medical records of patients who presented with a sudden painful and red eye while using eye drops or ointments were reviewed. Patients with erosion caused by contact between the tip of the bottle and their conjunctiva or cornea were included in the study. Patients were asked to demonstrate how they apply their eye drops, and inadvertent contact between the tip of the topical agents and the conjunctiva or cornea was observed. Results:Erosion on the conjunctiva or cornea as a result of direct contact of the tube or bottle tip was detected in 6 eyes of 5 pa tients. In 5 eyes, the erosion was at the lower bulbar conjunctiva with an outline resembling a map, and in 1 patient, erosion was at the 7 o’clock quadrant of the cornea and conjunctiva of right eye. The vision of the last patient was light perception, and the patient was on anti-glaucomatous medications. One patient was using eye drops after cataract surgery, and 3 patients were using ophthalmic ointments. The conjunctivae of all patients demonstrated staining and localized hyperemia in the same area, and in 1 patient, in addition to the corneal erosion, devel opment of infectious keratosis was observed. Eye patch and lu brication were used in the management of conjunctival erosions. Conclusion: Patients should be instructed in using the drugs at a distance from the eye. Especially in patients with impaired vision, instillation by another person should be recommended. Physicians should consider improper application of ophthal mic medications in the case of acute-onset localized eye red ness and pain during the usage of topical medications.
