Özet

Amaç: Postmenopozal dönemde HRT kullanan hastalarda antilipidemik bir ajan olan Simvastatin'in tedaviye eklendiği ve eklenmediği hasta ve kontrol grubunda serum lipidlerinin dağılımını incelemek. Materyal-Metod: Çalışmaya, total kollesterol seviyesi 200 mg/dl ve trigliserid seviyesi de 160 mg/dl üzerinde olan 124 postmenapozal hasta dahil edilmiştir. Hastaların başlangıç lipid profilleri çıkarıldıktan sonra 1. Grupta yer alan 62 hastaya HRT + Simvastatin başlanırken; 2. Grupta yer alan 62 hastaya ise sadece HRT başlanmıştır. Uygulanacak HRT protokolüne görede, 1. grupta saf östrojen preparatları, 2.grupta kontinü preparatlar, 3. grupta siklik preparatlar, 4. grupta TTS ve 5. grupta da Tibolon verilmiştir. Tedavinin başlangıcı 0. ay olarak kabul edildi ve ilk başlangıcından itibaren 0.,2.,4. ve ö.aylarda olmak üzere dört kez kan örnekleri alındı ve ölçümler yapıldı. Sonuçlar: HRT'nin tek başına ve da Simvastatin ile kombine edilmesiyle, kan lipidlerindenT.Kolesterol, Trigliserid, LDL ve VLDL değerlerinde düşme ve HDL değerlerinde yükselme sağlamıştır. Ancak sadece T.Kollesterol ve LDL deki düşüşler SMV li grupta istatistiki olarak anlamlı düzeyde olmuştur. Farklı HRT'ler gözönüne alındığında istatistiki olarak anlamlı bir fark elde edilememiştir. Tartışma: Sonuç olarak T.Kolesterol, Trigliserid, LDLve VLDL düzeylerinde düşme ile kardiyoprotektif özellikteki HDL değerlerinde yükselme sağlayan HRT ile kardiyovasküler risk faktörleri en aza indirilebilecek ve postmenopozal kadınlara kaliteli bir yaşam sunma fırsatı doğacaktır. HRT protokolüne eklenecek bir HMG-CoA redüktaz inhibitörü (Simvastatin) bu sonuca katkıda bulunuyor gibi görünmektedir. Ancak kesin sonuç için daha geniş çalışmalar ihtiyaç vardır.

Abstract

Objective: To compare the effectiveness of various hormone replacement agents with or without symvastatine (SMV) on lipid panel among postmenopausal women. Material and Methods: A total of 124 subjects included into this study. The subjects were not menstruate for at least 12 months and hormonally they were in postmenopausal period. All subject were divided into 2 main groups; Group 1: subjects that were used HRT+SMV and Group 2: subjects that were used HRT only. The HRT protocol was also divided into groups such as, Group 1: subjects that were used estrogen only; Group 2: subjects that were used continuous combined HRT; Group 3: subjects that were used cyclic HRT; Group 4: subjects that were used transdermal estrogens and Group 5: subjects that were used tibolone. The lipid panel of all subjects calculated at the beginning and 2,4,6 months afterwards. Results: When we compare the lipid results; we found that T.Chollesterol, LDL, VLDL and triglycerides were decreased and HDL was increased in all groups. But only the differences on T.Chollesterol and LDL were statistically significant among the two groups. Whereas we found no statistically significant differences between various HRT regimens. Conclusions : As a result we can say that HRT can be used for prevention of postmenopausal cardiovascular risk by decreasing T.Chollesterol, triglycerides and LDL and increasing HDL. Altough we found an additional benefit with SMV the efficacy of SMV must be scrutinized with further studies.
