Nifepidin kullanan preeklamptik gebelerde neonatal sonuçlar

Özet
Amaç: Antihipertansif tedavi amacıyla nifedipin verilen preeklamptik gebelerde perinatal sonuçları araştırmaktır. Materyal ve Metod: Üçüncü basamak hastanede yatan ve tedavisi yapılan 416 preeklamptik hasta retrospektif olarak incelendi. Doğum eylemi öncesinde veya sırasında, akut hipertansif atak tedavisi için kısa süreli veya kronik tedavi için uzun süreli nifedipin kullanan 178 hasta çalışma grubunu, antihipertansif hiçbir tedavi almayan 238 gebe kontrol grubunu oluşturdu. Her iki grup akut fetal distres, intrauterin ve postpartum ölüm oranları, yenidoğan hipoglisemisi, yenidoğan hipokalsemisi, yenidoğan sepsisi, yenidoğan trombositopenisi, yenidoğan konvülzyonu, yenidoğanda intrakraniyal kanama, sarılık ve yenidoğan yoğun bakım ihtiyacı açısından karşılaştırıldı. İstatistiksel analizde "student's t-test'1, Ki-kare test ve "backward conditional" lojistik regresyon analizi kullanıldı. Bulgular: Neonatal ve perinatal fetal sonuçlar açısından "univariate" analizde, nifedipin kullanan hasta bebeklerinde yenidoğan yoğun bakım ihtiyacı, postnatal ölüm oranı, sepsis oranı, intrakranial kanama ve sarılık oranları anlamlı olarak yüksek saptandı (sırasıyla, pO.0001, p=0.048, p=0.005, pO.0001 ve p=0.008). Lojistik regresyon analizinde ise nifedipin kullanımının hiçbir neonatal sonucu kötü olarak etkilemediği, aksine yenidoğan trombositopeni riskini ise azalttığı (p=0.04) saptandı. Sonuç: Preeklamptik kadınlarda kısa veya uzun süreli nifedipin kullanımı, perinatal morbidite ve mortalite artışına yol açmamaktadır. Aksine yenidoğan trombositopeni riskini azaltıyor olabilir.
Abstract
Objective: Tö investigate the perinatal outcomes in preeclamptic pregnancies receiving nifedipine as antihypertensive treatment Materials and methods: Four hundred and sixteen women admitted to a tertiary care unit with a diagnosis of preeclampsia were retrospectively included. Women given nifedipine for either acute hypertensive attack or long-term treatment of hypertension (n=178) were compared with controls, who did not receive any antihypertensive medication (n=238). The outcome variables included neonatal morbidity including admission to intensive care unit, hypoglycemia, hypocalcemia, sepsis, thrombocytopenia, convulsion, intracranial hemorrhage, jaundice, and polycythemia. Student's t-test, chi-square tests, and logistic regression with backward elimination were used for statistical analyses. Results: In univariate analyses, requirement for intensive care, neonatal mortality, sepsis, intracranial hemorrhage, and jaundice were more frequent in infants of women using nifedipine (pO.0001, p=0.048, p=0.005, pO.0001, and p=0.008, respectively). Logistic regression analysis revealed that nifedipine administration was not associated with increased risk of any of the outcome variables, but decreased the risk of developing neonatal thrombocytopenia (p=0.04) Conclusion: Short- or long-term nifedipine administration in preeclamptic women is not related with subsequent neonatal morbidity and mortality. Risk of neonatal thrombocytopenia may decrease following nifedipine use.
