Özet

Amaç: Unrüptüre ektopik gebeliklerin intramüsküler methotrexate ile tedavisi yaygın ve nisbeten ucuz bir tedavi şeklidir. Tek doz ve multidoz olarak iki farklı tedavi protokolü kullanılmaktadır. Çalışmanın amacı multi doz ya da tek doz methotrexate protokolü uygulanan 56 ektopik gebelik olgusundaki başarı oranlarını karşılaştırmaktır. Materyal ve Metod: Hasta bilgileri resrospektif olarak toplandı. Çalışma grubu medikal tedavi kriterlerine uyan 56 unrüptüre ektopik gebelik olgusundan oluşmakta idi. Hasta seçme kriterlerinde hemodinamik stabilitesi olan ve ultrasonografıde 4 cm den küçük ektopik gebelik kitlesine sahip olgular esas alındı. Ultrasonografık olarak fetal kardiyak aktivite gösterilen olgular çalışma dışında bırakıldı. Tek doz (çalışma grubu) ya da multipl doz (kontrol grubu) methotrexate intramüsküler olarak uygulandı. Her iki grupta demografik özellikler, gebelik yaşlan, serum hCG, progesteron düzeyleri, ektopik gebelik kesesi ölçüleri, ektopik gebelik öyküsü ve sonuçlar karşılaştırıldı. Bulgular: Tek doz protokolü uygulanan 44 hastanın 36'smda (%81.8) tedavi başarılı kabul edildi. Multipl doz protokolü uygulanan 12 hastanın 11'i (%91.6) tedaviye cevap verdi. Abstract

Objective: Medical treatment of an unruptured ectopic pregnancy with intramuscular methotrexate is a common practice and rather cost-effective. Two different treatment protocols, the "single dose" and the "multidose," have been advocated. The objective of this study was to compare the success rates in 56 patients with ectopic pregnancy who were treated with either multidose or single-dose methotrexate protocols. Materials and Methods: Data were collected retrospectively. The study population included 56 patients presenting with unruptured ectopic pregnancies who fulfilled the criteria for medical management. The selection criteria were patients who had a stable hemodynamic status and an ectopic gestational mass of 4 cm on ultrasonography. Patients were excluded from methotrexate therapy by the presence of fetal cardiac activity demonstrated on ultrasonography. A single dose (study group) or multiple doses (control group) of methotrexate were administered IM. We compared demographics, gestational age, serum human chorionic gonadotropin, progesterone levels, ectopic sac size, history of ectopic pregnancy, and outcome in both groups. Results: Of the 44 patients on the single-dose protocol, treatment was considered successful in 36 patients (81.8%).
