Kombine spinal epidural anestezi ile gerçekleştirilen sezaryenlerde hipotansiyon gelişimi ve efedrin kullanımı

Özet
Amaç: Sezaryende kombine spinal-epidural anestezi uygulaması sonrasında gelişen hipotansiyonda efedrin kullanım miktarını ve etkinliğini araştırmaktır. Materyal ve Metod: Bu çalışma ASA risk grubu I-II, kombine spinal epidural anestezi uygulanan 135 sezaryen olgusunda retrospektif olarak yapıldı. Olguların demografik verileri ile kalp atım hızlarının, sistolik, diastolik, ortalama arteryel kan basınçlarının ve periferik oksijen satürasyonlarının anesteziden önce, cerrahi süresince 5., 10., 15., 20., 30., 60., 90. 120. dakikalardaki değerleri ve hipotansiyon gelişen olgularda efedrin kullanım zamanlan ile dozları anestezi takip fişlerinden kaydedildi. Bulgular: Çalışmaya alınan 135 olgunun 104'ünde (%77) spinal anesteziyi takiben 0-10 dakika arasında hipotansiyon görüldüğü saptandı. Hipotansiyon gelişen olgulara toplam efedrin uygulaması dağılımları; olguların 30' unda (%22,2), 10 mg; 44' ünde (%32,6), 20 mg; 21'inde (%15,6), 30 mg; 9'unda (%6,7), 40 mg ve daha fazla efedrin uygulanarak hipotansiyonun önlendiği belirlendi. Sonuçlar: Sezaryen anestezisinde spinal-epidural uygulamayı takiben 0-10 dakikada gelişen hipotansiyonu önlemede zamanında yapılan efedrinin tedavide etkili olduğu kanısına varıldı.
Abstract
Objective: To investigate the dose and effectiveness of ephedrine for prevention of hypotension during combined spinal-epidural anaesthesia for caesarean delivery. Materials and Methods: This retrospective study included 135 ASA I-II patients undergoing caesarean section under combined spinal-epidural anaesthesia. Data on demographics, systolic and diastolic blood pressures, mean arterial blood pressure and SpO2 measured before anaesthesia and 5 10, 15, 20, 30, 60, 90, and 120 minutes after anaesthesia, the dose of ephedrine and the time of administration were obtained from patient observation forms. Results: Of 135 patients, 104 (77%) developed hypotension within 0-10 minutes of spinal anaesthesia. The average ephedrine dose required for prevention of hypotension was 10 mg in 30 (22,2%), 20 mg in 44 (32,6%), 30 mg in 21 (15,6%) and > 40 mg in nine patients (6,7%). Conclusion: To conclusion, ephedrine administered at the right time can prevent hypotension during combined spinal-epidural anaesthesia for caesarean delivery.
