Postmenapozal hormon replasman tedavisinin kardiovasküler risk belirteçleri ve brakial arterde akıma bağlı dilatasyona etkisi

Özet
Amaç: Bu çalışmanın amacı değişik ilaçlarla ve dozlarla hormon replasman tedavisi alan postmenapozal kadınlarda tedavinin lipid profili, akut faz reaktanları ve endotel fonksiyonları üzerindeki etkilerini incelemektir. Materyal Mcrod: Sağlıklı 105 postmenapozal kadın rastgele 4 gruba ayrıldı. 3 ay süre ile; 1.gruba (n:24) 1 mg l7betaestradiol/2mg drospirenon devamlı-kombine, 2. gruba (n:26) 1 mg estradiol hemihidrat/ 0,5 mg noretisteron asetat, devamlı-kombine, 3.gruba (n:27) 0,625 mg konjuge ekin estrojen/2,5 mg medroksiprogesteron asetat, devamlı-kombine, 4gruba (n:28) herhangi bir ilaç verilmedi. Olgular yaş, ağırlık, boy, vücut kitle indeksi, menopoz süresi, sigara alışkanlığı, nabız ve tansiyon arteriyel gibi klinik ve epidemiyolojik özellikler ve serum FSH, estradiol, CRP, Homosistein, total kolesterol, trigliserid, HDL ve LDL gibi laboratuar parametreleri açısından araştırıldılar. Ayrıca endotel fonksiyonu değerlendirmek için her hastada bazal ve 3 aylık tedavi sonrası brakial arterde 3 ölçüm gerçekleştirildi. Sonuçlar: 1. ve 2.gruptaki homosistein artışı, 2. ve 3. grupta CRP artışı saptandı, bazal arter ölçümü ve Nitrogliserin sonrası brakial arter ölçümü 4 gruptada literatürle uyumlu olarak farklı çıkmadı. Reaktif hiperemideki brakial arter ölçümü I. ve 2. grupta anlamlı çıktı. Bu bulgu düşük doz HRT'nin endotel fonksiyonları üzerine olumlu etkisi olduğu fikrini destekliyor. 3. ve 4. grupta anlamlı değişiklik saptanmadı. Lipid profilinde ise 3.grupta total kolesterol ve LDL'ııin azalmış bulundu. Yorum: Menopozda HRT'nin lipid profili ve endotel fonksiyonları üzerinde yararlı etkileri görülmüştür. Bu etkiler düşük doz estrojen alan grup içinde geçerlidir. Ancak sonuçlar HRT'nin daha titiz bir inceleme sonrası, daha seçici verilmesi gerektiğini göstermektedir.
Abstract
Introduction : The aim of the present study was to evaluate the lipoprotein profile and endothelial function in postmenopausal women receiving hormone replacement therapy. Materials-Method : 105 postmenopausal women were randomly divided into four groups. Oroup 1 (n:24) recieved 1 nig 17 beta estradiol +2mg drospirenon /day, Group 2 (n:26) recieved 1 mg estradiol hemihydrate+0,5 mg norethisteron acetate /day, Group3 (n:27) 0,625 mg Conjugated Equine estrogen +2,5 mg medroxiprogesteron acetate / day for 3 months and the Group 4 (n:28) was the control group. Serum estradiol, FSH , CRP, homocysteine, total cholesterol, trigliserid, HDL-C and LDL-C were measured at the beginning and at the end of third month of HRT in every patient. For endothelial functions brachial artery (BA) diameter and blood How velocity at rest and after occlusion by high pressure cuff were also measured at the beginning and at the end of the third month. Results: Total and LDL cholesterol were found to be decreased in Group 1,2 and 3 with HRT. Homocysteine in Group 1 and 2, CRP in Group 2 and 3 were found to be increased after 3 months of HRT.Increase in the brachial artery diameter and blood flow velocity after occlusion by high pressure cuff in Group 1 and 2 was significant at the end of third month of treatment. Conclusion: HRT improved endothelial function and reduced plasma levels of lipids in postmenopausal women . These beneficial effects were also found in low dose estrogen group.
