Özet
Endometriyal hiperplazi, en sık jinekolojik malignensi olan endometrial kanserin öncü lezyonudur. En sık belirtisi anormal vajinal kanamadır. Endometriyal hiperplazi her yaşta görülmekle birlikte, en sık postmenopozal hastalarda görülür. Endojen veya eksojen estrojen miktarını artıran tüm sebepler birer risk faktörüdür. Transvajinal ultrason ciddi endometriyal patolojiyi ekarte etmekte oldukça başarılı iken, endopipelle ile endometriyal biyopsi de bu durumu teşhis etmekte oldukça başarılıdır. Bu iki metod postmenopozal kanama ile başvuran hastalarda ilk teşhis yöntemleri olarak kullanılmalıdır. Sürekli kombine hormon tedavisi endometriyal hiperplazi riskini artırmaz; ancak ardışık hormon tedavisi 12 ay sonunda endometrial hiperplazi riskini artırır. Endometriyal hiperplazide tedavi seçenekleri hastanın yaşına, fertilite arzusuna, cerrahi risk faktörlerine ve atipi varlığına göre belirlenir. Atipisiz endometrial hiperplazi progestin tedavisine genellikle iyi cevap verir. Atipili hiperplazide, hastalığın ilerlemesi veya aynı anda endometriyal karsinom bulunma riski yüksektir; bu nedenle cerrahiye engel olacak risk faktörleri ve şiddetli fertilite koruma isteği olmadığı takdirde tedavi histerektomi olmalıdır.
Abstract
Endometrial hyperplasia is the precursor of endometrial carcinoma, which is the most common gynecologic malignancy. Most common presenting symptom is abnormal vaginal bleeding. Endometrial hyperplasia can be seen at any age; however it is more common in postmenopausal women. Predisposing factors are mostly factors that cause increased estrogen stimulation of the endometrium. Transvaginal ultrasound is highly successful in excluding serious endometrial pathology, and endopipelle endometrial biopsy is highly successful in diagnosing it. These two diagnostic methods should be employed in the initial assessment of abnormal bleeding in postmenopausal women. Continuous combined hormone therapy does not increase the risk of endometrial hyperplasia; however, sequential hormone therapy increases the risk at 12 months. Treatment choice in endometrial hyperplasia is dependent upon patient age, desire for future fertility, surgical risk and whether there is atypia. Endometrial hyperplasia without atypia generally responds well to progestins. However, there is a high risk of progression or concurrent carcinoma in atypical hyperplasia, thus it should be treated with hysterectomy unless there is strong desire for fertility or comorbidity precluding surgery.
