How Compatible is Clinical Diagnosis with Electrophysiology?

Özet
Amaç: Bu çalışmanın amacı, elektronöromiyografi laboratuvarına incelemeamacıyla gönderilen hastaların klinik ön tanısı ile elektronöromiyografi tanısının uygunluğunu araştırmaktı. Gereç ve Yöntem: Elektronöromiyografi laboratuvarına gönderilen 882 hastanın yaş, cinsiyet, gönderilen klinik, klinik öntanı ve elektronöromiyografi işlemi sonuçları retrospektif olarak kaydedildive istatistiksel olarak değerlendirildi. Bulgular: Bu çalışmaya 882 hasta (524kadın, 358 erkek) dahil edildi. 499 (%56,6) hastanın ENMG istemi Nöroloji,222 (%25,2) hastanın elektronöromiyografi istemi Nöroşirurji, 59 (%6,7) hastanın elektronöromiyografi istemi Fiziksel Tıp ve Rehabilitasyon, 61 (%6,9)hastanın elektronöromiyografi istemi Ortopedi ve 41 (%4,6) hastanın elektronöromiyografi istemi ise diğer bölümlerden yapılmıştı. Karpal tünel sendromu %28,7 ile en çok istem yapılan ön tanıydı. Hastaların %2,9u gibi küçük bir grubunda sadece semptom belirtilerek tetkik istemi yapılmıştı. Hastaların %49,1inde istem ön tanısı ile elektronöromiyografi sonucu arasındauyum saptandı. %44,7 hastada normal elektronöromiyografi sonuçları bulundu. Tartışma: Normal elektronöromiyografi bulgularının yüksek oranı ve istemön tanılarıyla elektronöromiyografi sonuçlarının istatistiksel olarak anlamlıuyumsuzluğu, gereksiz elektronöromiyografi istemlerini ortaya koymaktadır.
Abstract
Aim: The objective of this study was to investigate the correlation of the clinical diagnosis of patients referred to the electroneuromyography laboratorywith the electrophysiological diagnosis. Material and Method: Eight hundredeighty two patients were enrolled in the study. Their ages, genders, the clinicthat referred the patient, the clinical diagnosis, and the electrophysiologi- cal results were registered retrospectively and the results were statisticallyevaluated. Results: Five hundred twenty four of 882 patients enrolled in thestudy were women, while 358 of them were men. Four hundred hundred nine- ty nine (56,6%) of the patients were consulted by the Neurology department,222 (25,2%) patients were consulted by the Neurosurgery deparment, 59(6,7%) patients were consulted by the Physical medicine and rehabilitationdeparment while 61 (6,9%) patients were referred by the Orthopedy department and 41 (4,6%) by other departments. Carpal tunnel syndrome was themost frequent clinical diagnosis (28,7%). Only in a small group of patientssymptoms were assessed rather than the clinical diagnosis (2,9%). In 49,1%of the patients, There was correlation between clinical and electrophysiological diagnosis while 44,7% of patients had normal electroneuromyographyresults. Discussion: The high rate of the normal electroneuromyography testsand the statistically significant discordance of clinical diagnosis of patientswith their electrophysiological test results show that, there are inessentialelectroneuromyography reguests.
