Şanlıurfada tahıl polen duyarlılığı ve astım kliniğine etkisi

Özet

Amaç:Çalışmamızın amacı astımlı olgularımızda deri prick testleri ile tahıl polenlerine karşı duyarlılığı belirlemek ve farklı tahıl polen duyarlılıklarının astım kliniğine etkisini araştırmaktı. Gereç ve Yöntem: Bu çalışmada OSM Ortadoğu Hastanesi Göğüs Hastalıkları polikliniğine Şanlıurfa il merkezinden astım tanısı ile Mart-Haziran aylarında başvuran ve deri prick testlerinde tahıl polenlerine karşı duyarlılığı bulunan, yaş ortalamaları 26.3-7.9 yıl olan, 83’ü kadın, 57’si erkek 140 olgu prospektif olarak değerlendirildi. Olgular 16 hafta süreyle sabah ve akşam ölçülen tepe ekspiratuvar akım hızı (PEF), PEF değişkenliği, astım semptom skorları ve çalışma boyunca kullandıkları semptom rahatlatıcı tedavilerini kaydeden hastalar; başlangıç, 1,2,3 ve 4. ay sonunda klinikte FEV1 sonuçları ile değerlendirildi. Bulgular: Olgularda %25.4 Hordeum vulgare, %23.6 Secale cereale, %20.7 Triticum sativum %.19.3 Zea mays, % 18.6 Avena sativa polenlerine karşı duyarlılık saptandı. Çalışma öncesi ve sonrası sabah ve akşam PEF değerleri, astım semptom skorları ve semptom rahatlatıcı ilaç kullanımı olgularda benzerdi (p>0.05). Sonuç: Şanlıurfa ilinin bitki örtüsü ile uygun olarak deri prick testleri ile Hordeum vulgare, Secale cereale, Triticum sativum, Zea mays, Avena sativa polenlerine karşı önemli oranda duyarlılık saptandı. Tahıl polenlerinin astım kliniğine etkileri benzer oranlarda gözlenmiştir.

Abstract

Aim:Our aim in this study was to detect the kinds of cereal that cause cereal pollens sensitivity, and to investigate the effects of cereal pollen sensitivity on the asthma clinic. Material and Method: In this study 140 cases from Sanliurfa with asthma (83 women, 57 men, with the mean age of 26.3-7.9 year) and sensitivity against cereal pollen demonstrated by skin prick test, who applied to OSM Ortadogu Hospital Chest Diseases Outpatient Clinic in the period of March-June were evaluated prospectively. The patients’ peak expiratory flow (PEF) values measured in the morning and evening, changes in PEF, asthma symptom score and the symptom-relieving therapy used during the 16-weeks study period were recorded and evaluated in the clinic with FEV1 results at the beginning, month 1, 2, 3 and 4. Results: Of the patients with sensitivity against cereals pollens, 25.4% had allergy against Hordeum vulgare, 23.6% against Secale cereale, 20.7% against Triticum sativum, 19,3% against Zea mays and 18.6% against Avena sativa. Prior to the study and at the end of follow up period, morning and evening PEF values, asthma scores and symptom relieving drug usage were same cereals pollens sensitivity (p>0.05). Discussion: Accordance to the vegetation of Sanliurfa region, important ratio of the sensitivities were detected with prick test against Hordeum vulgare, Secale cereale, Triticum sativum, Zea mays and Avena sativa pollens. The effects of cereals pollens were similar rates of asthma clinics.
