A Rare Cause of Acute Abdomen: Urinary Bladder Rupture Due to Foley Catheter

Özet
Mesane ruptürü sıklıkla künt veya penetran travmalar, yada iatrojenik yaralanmalar sebebiyleolup spontan intraperitoneal perforasyonu oldukça nadir görülmektedir. 57 yaşındaki erkek hastada karın ağrısı, bulantı-kusma ve yüksek ateş şikayetinin gelişmesi üzerine genel cerrahikonsültasyonu istenmiştir. Özgeçmişinde diabetes mellitus, multipl skleroz, Lerichesendromu, 5 yıl önce benign prostat hipertrofisi nedeniyle operasyon öyküsü mevcut olup sıksık foley sonda kateterizasyonuihtiyacı duyuluyormuş. Foley sonda olgunun kendisitarafından nonsteril şartlardatakılıyormuş. Fizik muayenede sağ alt kadranda daha belirgin olmak üzere supra- pubik defans ve rebaund saptandı. Olgu akut batın (Akut perfore apandisit)ön tanısıyla operasyona alındı. Yapılan explorasyonda mesanenin tamamının duvarınıtamkat tutan flegmon tarzında infeksiyon , yer yer apse odakları, küçük bir alanda nekroz vemetilen mavisi yardımıyla tespit edilen mesanede mikroperforasyonsaptandı. Olguyaintraabdominal apsenin drenajı, sistostomi, mesaneye primer sütür operasyonu uygulandı.Olgu postoperatif 9.gün sepsis nedeniyle exitus oldu.
Abstract
Urinary bladder rupture is usually found due to blunt or penetrating traumas andiatrogenic injuries, and spontaneous intraperitoneal perforation is very rare. 57years old male patient was consulted to general surgery department due to ab- dominal pain, nausia-vomiting and fever. He had Diabetes Mellitus, Multiple Sclerosis, Leriche Syndrome and operation for benign prostatic hypertrophy 5 yearsago on his medical history which the case catheterised him urinary bladder him- self frequently in nonsterile conditions. He did not care about sterility. On physical examination, suprapubic defence and rebound was noted more prominentlyon right lower quadrant. He was taken to operating room with the diagnosis ofacute abdomen. On exploration, infection was seen on all layers of the whole urinary bladder, abscess formation and necrosis causing a microperforation which isdetected by methylene blue was also present. Intraabdominal abscess drainage,cystostomy and primary suturing of the urinary bladder was performed. Patientdied due to sepsis at the 9.th post operative day.
