Radical Prostatectomy and Active Surveillance in Prostate Cancer; The Evaluation of Erectile Function and Depression

Özet

Amaç: Düşük riskli prostat kanserinde küratif tedavinin yanında aktif izlemseçeneği de mevcuttur. Ancak bu hastalarda tedavi edilmemiş kanser ile yaşanmasına bağlı olarak anksiyete ve stres olabilir. Bu çalışmanın amacı lokalize prostat kanserli hastalarda aktif izlem veya radikal prostatektomi yapılmasının erektil işlev ve depresyon skorları üzerine etkisinin olup olmadığını araştırmaktır. Gereç ve Yöntem: 2007 2010 yılları arasında GATA Haydarpaşa Eğitim Hastanesi Üroloji kliniğinde yapılan tetkikleri sonrasında lokalize prostat kanseri tanısı alan hastalardan aktif izlem protokoluna alınanlar ile benzer ameliyat öncesi klinik ve patolojik özelliklere sahip olan radikalprostatektomi yapılmış hastalar karşılaştırıldı. Her iki gruptaki hastaların yaş,PSA, klinik evre, pozitif kor sayısı, Gleason skoru, tanı ile değerlendirme zamanı arasındaki süre kaydedildi. Hastalarla yüzyüze görüşülerek Beck depresyon ölçeği ve IIEF (International Index of Erectile Function) dolduruldu. Bulgular: Aktif izlem grubunda 18, radikal prostatektomi grubunda 17 hasta vardıve ortalama takip süreleri sırasıyla 16,8 (3 41), 59,35(51 71) yıldı. Grupların yaş, PSA değerleri ve Gleason skorları benzerken, pozitif kor sayısı ve klinik evreleri birbirinden farklıydı (Tablo 1). Aktif izlem grubundaki hastalarınIIEF skorları 1 30 arasında, radikal prostatektomi grubunda ise IIEF skorları 1 28 arasındaydı ve her iki grup arasındaki farklılık istatistiki olarak anlamlı değildi (p=0.203). Beck Depresyon ölçeği skorları ise aktif izlem grubunda 0 24 arasında değişirken radikal prostatektomi grubundaki 0 16 arasındaydı ve grupların Beck Depresyon ölçeği skorları benzerdi (p=0.157). Sonuç:Radikal prostatektomi ile aktif izlem arasında erektil işlev bakımından farklılık yoktur. Depresyon semptomatolojisi açısından da farklılığın olmaması aktif izlemin hastalara ek bir psikolojik stres yüklemediğini düşündürmektedir.

Abstract

Aim: The aim of this study is to investigate whether active surveillance orradical prostatectomy in patients with localized prostate cancer has effecton erectile function and depression scores. Material and Method: Betweenyears 2007 and 2010, patients diagnosed with localized prostate cancerafter tests and received an active surveillance protocol and patients whounderwent radical prostatectomy with similar preoperative clinical and path- ological features were compared. Age, PSA, clinical stage, number of positivecores, Gleason score, time interval between diagnosis and the assessmentof the patients in both groups were recorded. Beck depression scale andIIEF were filled by face to face interviews with patients. Results: There were18 patients in active surveillance group, 17 patients in the radical prosta- tectomy group and mean follow-up periods were 16.8 (3-41), 59.35 (51-71)years, respectively. While age, PSA levels and Gleason scores were similar,the number of positive cores and clinical stages were different from eachother. IIEF scores of patients in the active surveillance group were between1-30, IIEF scores in the radical prostatectomy group were between 1-28,and the difference between the two groups was not statistically significant(p = 0.203). Beck Depression Scale scores ranged from 0-24 in the activesurveillance group, and 0-16 in the radical prostatectomy group and the BeckDepression Scale scores were similar between groups (p = 0.157).Discussion:There is no difference between radical prostatectomy and active surveillancein terms of erectile function. Because of the lack of difference in terms ofsymptomatology of depression suggests that active surveillance does notload an additional psychological stress to patients.
