Outcomes of the Modified Two-Patch Technigue in Patients with Complete Atrioventricular Septal Defect

Özet
Amaç: Bu çalışmada komplet atriyoventriküler septal defektli (KAVSD) hastalarda modifiye çift yama tekniğinin sonuçlarının bildirilmesi amaçlandı. Gereç ve Yöntem: Eylül 2005- Ekim 2011 tarihleri arasında kliniğimizde komplet AVSD tanısı konulan ve two-patch tekniği ile cerrahi tamir yapılan 50 hasta (40 kız, 10 erkek; ort.yaş13.64±15.72 ay) retrospektif olarak değerlendirildi. Tüm hastalara standart anestezi, cerrahi ve KPB protokolleri uygulandı. Demografik veriler, intraoperatif ve postoperatif sonuçlar kaydedildi. Bulgular: Hastaların 46 tanesi Rastelli tip A (%92), 1 tanesi Rastelli tip B (%2) ve diğer 3 tanesi Rastelli tip C (%6) idi. Kardiyopulmoner baypass süreleri, 115.42 ±39.171 dakika ve aortik klemp süreleri 84.44 ±31.155 dakika idi. Ameliyat sonrası ekokardiografik incelemede bir hastada (%2) sol atriyoventriküler kapakta orta-ileri yetmezlik tespit edildi ve mitral plasti yapıldı.3 hastada sol atriyoventriküler kapakta hafif-orta kapak yetmezliği görüldü. Hastalarda sol ventrikül çıkım yolu darlığı görülmedi. Atriyoventriküler tam blok gelişen bir hastada (%2) ritim kendiliğinden düzeldi. Ameliyat sonrası erken dönemde 2 hasta (%4) kaybedildi. Sonuç: Komplet atriyoventriküler septal defektli hastalarda modifiye çift yama tekniği kabul edilebilir sonuçlarla uygulanabilir.
Abstract
Aim: This study aims to reported the outcomes of the modified twopatch technique in patients with complete atrioventricular septal defects(AVSD). Material and Method: Fifty patients (10 males, 40 females; meanage13.64±15.72 months) who were diagnosed with AVSD and underwent two- patch technigue with surgical repair between September 2005 and October2011 in our clinic were retrospectively evaluated. Standard anesthetic,surgical, and CPB protocols were used in all patients.The demographicdata, intraoperative and postoperative outcomes were recorded. Results:Rastelli type A in 46 patients (92%), Rastelli type B in 1 patient (2%), Rastellitype C in 3 patients (6%) were seen. Cardiopulmonary bypass and aorticclamp times were 115.42 ±39.1minutes and 84.44 ±31.1, respectively.Postoperative moderate-severe degree of left-sided atrioventricular valveinsufficiency was observed in the echocardiographic evaluation of onepatient (2%) and we performed the mitral annuloplasty. A moderate degreeof left-sided atrioventricular valve insufficiency was observed in threepatients. Left ventricular outflow tract obstruction was not detected in ourpatients. One patient (2%) developed complete atrioventricular blockage inthe postoperative period and rhythm spontaneously improved. There weretwo (4%) deaths in the early postoperative period. Discussion: The modifiedtwo-patch technique can be performed with acceptable results in infantswith complete AVSD
