Comparison of the use of platinum-based combination in the first and second line treatment of non-small cell lung cancer

Özet
Amaç: Küçük hücreli dışı akciğer kanserinde ilk basamak tedavide kullanılan kemoterapi kombinasyonlarının etkili olmalarına rağmen hastaların çoğunluğunda takiplerde hastalık progresyonu meydana gelmektedir ve bu yaşam süresini kısaltmaktadır. İkinci basamak tedavide sıklıkla sitotoksik kemoterapiler tek ajan olarak kullanılmaktadır Bu retrospektif çalışmada; küçük hücreli dışı akciğer kanserinde ilk basamak tedavide gemsitabin-sisplatin, ikinci basamak tedavide taksan kullanımı ile ilk basamak tedavide taksan-sisplatin, ikinci basamak tedavide gemsitabin kullanımının etkinliği karşılaştırılmıştır. Gereç ve Yöntem: Histopatolojik olarak küçük hücreli dışı akciğer kanseri tanısı almış ve görüntüleme yöntemleri ile evrelendirme çalışmaları yapılmış lokal ileri ve metastatik küçük hücreli dışı akciğer kanserli hastalar çalışmaya alınmıştır. Birinci guruba (Grup A) ilk basamak tedavide gemsitabin 1250 mg/m2 1+8. günler, sisplatin 75 mg/m2 1.gün oluşan kemoterapi kombinasyonunun 21 günde bir uygulandı. İkinci basamakta dosetaksel 75 mg/m2 veya paklitaksel 175 mg/m2 21 günde bir uygulandı. İkinci guruba (Grup B) ilk basamak tedavide dosetaksel 75 mg/m2 1. gün sisplatin 75 mg/m2 1.günden oluşan kemoterapi kombinasyonunun 21 günde bir uygulandı. İkinci basamak tedavi olarak gemsitabin 1250 mg/m2 1+8. Günler kullanıldı. Bulgular: Her iki kol birlikte değerlendirildiğinde hastaların medyan yaşam süresi 13,1 ay olarak tespit edildi. Grup A’da medyan yaşam süresi 14,8 ay saptanırken Grup B’de medyan yaşam süresi 12,9 ay olarak saptandı. Yaşam süreleri arasındaki fark long rank testi ile değerlendirildiğinde istatiksel anlamlı fark tespit edilmedi. Tartışma: Çalışmamızda gemsitabin-sisplatin kombinasyonu sonrası taksan kullanımı ile taksan-sisplatin kombinasyonu sonrası gemsitabin kullanımı kolları arasında fark araştırıldı ve genel sağkalım farkı gösterilemedi. Küçük hücreli dışı akciğer kanserinin ikinci basamak tedavisinde hastanın performans durumu, tümöre ait moleküler özellikler yanında almış olduğu ilk basamak tedavisinin de göz önünde bulundurulması gerektiğini düşünüyoruz.
Abstract
Aim: Although chemotherapy combinations used for the first stage treatment of the non-small cell lung cancer are effective, in the follow-ups of most of the patients, disease progression occurs which shortens the survival time. In second stage treatment, cytotoxic chemotherapies are frequently used as the single-agent. In this retrospective study, the effectiveness of use of gemcitabine-cisplatin in the first stage treatment of non-small cell lung cancer and of taxan in the second stage treatment was compared with that of use of taxan-cisplatin in the first stage treatment and of gemcitabine in the second stage treatment. Material and Method: Locally advanced and metastatic non-small cell lung cancer patients, who have been histopathologically diagnosed as non-small cell lung cancer and whose staging studies have been made by screening methods, have been involved in this study. For the first group (Group A), the chemotherapy combination consisting of the administration of 1250 mg/m2 gemcitabine on the days 1+8 and of 75 mg/m2 cisplatin on the 1st day was administered every 21 days as the first stage treatment. In the second stage, 75 mg/m2 docetaxel or 175 mg/m2 paclitaxel was administered every 21 days. For the second group (Group B), the chemotherapy combination consisting of the administration of 75 mg/m2 docataxel on the 1st day and of 75 mg/m2 cisplatin on the 1st day was administered every 21 days as the first stage treatment. As the second stage treatment, 1250 mg/m2 gemcitabine was used on the days 1+8. Results: When two treatments are evaluated together, median survival time of the patients was determined as 13,1 months. While for the Group A the median survival time was determined as 14,8 months, it was determined as 12.9 months for the Group B. When the difference between the survival times was evaluated by long rank test, no statistically significant difference was determined. Discussion: In our study, the difference between the use of taxan following the gemcitabine-cisplatin combination and use of gemcitabine following the taxan-cisplatin combination was investigated and no general survival difference could be demonstrated. We think that, in the second stage treatment of non-small cell lung cancer, along with the performance status of the patient and the molecular characteristics of the tumour, the first stage treatment he/she has received should also be taken into account.
