Role of Macrophage Microaggregation in the Diagnosis of Inflammatory Colitis

Özet
Amaç: Son dönemde yapılan çalışmalar, makrofaj mikroagregasyonunun(MMA) Crohn koliti (CC) tanısında kriter olabileceğini desteklemektedir. Buçalışmada MMAnun ülseratif kolit (UC) ve CCni ayırmadaki rolünü araştırdık. Gereç ve Yöntem: Bu çalışmada 29 UC hastası, 26 CC hastası ve 22 inflamatuvar barsak hastalığı olmayan sağlıklı bireyde MMAnın rolünü araştırdık. Bütün bireylerde özefagogastroduedonoskopi yapıldı. Mide ve duodenumdaki lezyonsuz bölgelerden biyopsiler alındı. Biyopsi materyalleri MMAiçin immünohistokimyasal olarak boyandı. Ayrıca serum örneklerinden ELI- SA yöntemi ile perinükleer antinötrofil sitoplazmik antikor (pANCA) ve anti- Saccharomyces Cerevisiae antikoru ASCA (immunoglobulin G ve A) bakıldı.Hastalarda ve kontrol grubunda histopatolojik olarak Helicobacter pylori (H.pylori) varlığına da bakıldı. Bulgular: MMA değeri CC ve UC hastalarında kontrol grubundan yüksek bulundu (sırasıyla % 46.2, % 42.3, % 9,1). Hastalarla kontrol grubu arasında MMA açısından istatistiksel olarak anlamlı bir farkvardı (p=0.007). CC ve UC hastaları arasında MMA açısından anlamlı bir farkyoktu (p>0.05). Tartışma: Hem UK hem de CK hastalarında MMA pozitifliğiartmaktadır. İnflamatuvar kolitli hastalarda H. Pylori varlığı, pANCA ve ASCApozitifliği sağlıklı kontrollerle benzer bulunmuştur.
Abstract
Aim: Recent studies have advocated that the presence of macrophage microaggregations (MMA) may be a criterion in the diagnosis of Crohn s colitis(CC). In our study we aimed to investigate the role of MMA to differentiate ulcerative colitis (UC) and (CC). Material and Method: We analyzed therole of MMA in 29 patients with UC, 26 patients with CC and 22 healthysubjects without diagnosis of inflammatory bowel disease. For all subjects,esophagogastroduodenoscopy was performed. Biopsies were taken fromnon-lesion regions of stomach and duodenum. Biopsy materials underwentimmunohistochemical staining for the microscopic investigation of the presence of MMA. Also, determination of Perinuclear Anti-Neutropil CytoplasmicAntibodies (pANCA) and Anti-Saccharomyces Cerevisiae Antibodies (ASCA)(Immunoglobulin G and A) was done with ELISA in serum samples. In patientand control groups, presence of Helicobacter pylori (H. pylori) positivity washistopathologically evaluated. Results: MMA was higher in patients with bothCC and UC compared with control groups (46.2%, 41.3%, and 9.1% respectively). There was statistically significant difference between patient groupsand the control group in terms of MMA but there was no difference betweenCC and UC groups (p=0.007). No statistically significant difference was obtained between the groups in terms of ASCA and p-ANCA. H.pylori positivitywas determined in 41.3% of MMA patients with CC, in 75% of patients withUC, and in 50% of healthy subjects. There was no significant difference between the three groups (p=0.344). Discussion: MMA positivity increases inpatients with both CC and UC. In patients with inflammatory colitis, H. pyloriexistence, pANCA and ASCA positivity was similar to healthy subjects.
