Distribution of Hepatitis C Virus Genotypes in the South Marmara Region

Özet
Amaç: Hepatit C virüsü, dünyada ve ülkemizde hepatit, siroz ve hepatosellüler karsinomun önemli bir nedenidir. Altı genotipi ve 100 civarında türümsü tanımlanmış olan HCV enfeksiyonun seyri ve tedaviye yanıtı genotip ileilişkilidir. HCV genotiplerinin bilinmesi epidemiyolojik verilerin belirlenmesi açısından da önemlidir. Bu çalışmada Güney Marmara Bölgesinin referans merkezi olan Uludağ Üniversitesi Tıp Fakültesi Hastanesi Mikrobiyoloji Laboratuvarına gönderilen HCV genotiplerinin belirlenmesi amaçlanmıştır. Gereç ve Yöntem: Çalışma Temmuz 2010- Aralık 2012 döneminde laboratuvarımıza genotipleme için gönderilen HCV enfeksiyonu olan hastaların serum örnekleri ile retrospektif olarak yapıldı. Çalışmada HCV RNA içinRotor-Gene Q (Qiagene, Hilden Germany) cihazında Artus HCV QS-RGQ PCRkiti (Qiagene,Hilden, Germany) kullanıldı. HCV RNA pozitif bulunan hastalarda Linear Array HCV genotyping test (Roche, NJ, USA) kiti ile HCV genotip çalışması gerçekleştirildi. Bulgular: Çalışmaya dahil edilen 231 hastadan214ü (%92.6) genotip 1, biri (%0.4) genotip 2, dokuzu (%3.9) genotip 3, yedisi (%3.1) genotip 4 olarak bulundu. Genotipi 3 olan hastalardan üçünün yabancı uyruklu, ikisinin ise yurt dışında doğmuş olduğu, genotip 4 olan hastalardan birisinin de yurt dışında doğmuş olduğu belirlendi. Tartışma: Ülkemizverileriyle uyumlu olarak en yaygın görülen genotipin 1 olduğu, genotip 3ünözellikle yurtdışıyla ilişkili hastalarda pozitif olduğu, genotip 4ün ise daha azgörüldüğü belirlendi. Ülkemizde ve bölgemizde en sık olduğu saptanan genotip 1in önemi ; kronik hepatit C hastalığında antiviral tedaviye direncin fazlaolması ve daha uzun süre tedavi gerektirmesidir.
Abstract
Aim: Hepatitis C virus (HCV) is an important caustive agent of hepatitis, cirrhosis and hepatocellular carcinoma both in our country and the world. Prognosis and response to treatment is related with the genotype of HCV whichhas six genotypes and over a hundred quasispecies. Knowing the HCV genotype is also important for epidemiological data. In this study we aimed toinvestigate the HCV genotypes of samples sent to Uludağ University HospitalMicrobiology Laboratory which is the reference centre in the South MarmaraRegion. Material and Method: This study was done retrospectively to analysethe HCV patients sera sent to our laboratory between July 2010and Decem- ber 2012 for HCV genotyping. Artus HCV QS-RGQ PCR kit (Qiagene,Hilden,Germany) was used in Rotor-Gene Q (Qiagene, Hilden Germany) for detectionof HCV RNA. HCV RNA positive samples of patients sera were were used forgenotyping by the Linear Array HCV genotyping test (Roche, NJ, USA).Results:214 (92.6 %) of total 231 patients included in the study were genotype 1,one (0.4 %) was genotype 2, nine (3.9 %) were genotype 3 and, seven (3.4%) were found genotype 4. Three of genotype 3 patients were of foreignnationality, two were born abroad and one of the genotype 4 patients wereborn abroad. Discussion: Concordant with our country data the most frequentgenotype was 1, genotype 2 was seen in patients especially related withforeign countries and genotype 4 was seen rare. The importance of genotype1, which is seen more frequent in our country and region is; resistance toantiviral treatment and prolonged treatment duration in chronic hepatitisC patients.
