The relationship between endometrial pathology and age in abnormal uterine bleeding: 1552 cases

Özet
Amaç: Bu çalışmanın amacı anormal uterin kanamada (AUK) yaş ile endometrial patoloji arasındaki ilişkiyi araştırmaktır. Gereç ve Yöntem: Bursa Şevket Yılmaz Eğitim ve Araştırma Hastanesi Kadın Hastalıkları ve Doğum Bölümünde AUK nedeni ile endometrial kürtaj ya da biyopsi yapılan 1552 hastaya ait medikal ve patolojik kayıtlar incelendi. Leimyomu, servikal, vajinal patolojisi ve hemostatik bozukluğu olan ve işlem öncesi gebelik tespit edilen olgular çalışmaya dahil edilmedi. Bulgular: Çalışma popülasyonunun yaş ortalaması 45.4 ± 9.3 idi. Yaştan bağımsız olarak en sık patern ve endometrial patoloji sırasıyla normal siklik endometriyum (%32.9) ve benign endometrial po-ip (%27.8) idi. AUK en sık olarak 41-50 yaş grubunda (%49.8) gözlendi ve bugrupta en baskın lezyon endometrial polip olarak saptandı. AUKsı olan post- menopozal hastalarda endometrial karsinom sıklığı %2.4 idi. Bu hastalardaen sık patoloji atrofik endometriyumdur (%51.2). Tartışma: Hastanın yaşı ileendometrial patolojinin tipi arasında güçlü bir ilişki bulunmaktadır. Anormaluterin kanamalı olgularda uygun tanı yöntemlerinin seçiminde hastanın yaşıdikkate alınmalıdır.
Abstract
Aim: The aim of this study is to investigate the relationship between endometrial pathology and age in abnormal uterine bleeding (AUB). Materialand Method: The medical and pathological records of 1552 patients thatendometrial curettage or biopsy was performed for AUB from at the Department of Obstetrics and Gynecology of Bursa Şevket Yılmaz Research andEducation Hospital were reviewed. Those with leiomyoma, cervical, vaginalpathology and hemostatic disorders and diagnosed as pregnant before theprocedure were excluded. Results: The mean age of study population was45.4 ± 9.3. The most common pattern and endometrial pathology regardlessof age was normal cycling endometrium (32.9%) and benign endometrial polyp (27.8%) respectively. AUB was most commonly seen in the 41 to 50 yearsage group (49.8%) and the predominant lesion noted was benign endometrialpolyp. The incidence of endometrial carcinoma was 2.4% in postmenopausalpatients with AUB. Atrophic endometrium (51.2%) was the most common pathology in those patients. Discussion: There is a strong correlation betweenthe type of endometrial pathology and age of patient. The age of patientshould be taken into account during the selection of appropriate diagnosticmethods in cases with AUB.
