A case of spontaneously resolved bilateral primary spontaneous pneumothorax

Özet

Herhangi bir travma hikayesi ya da akciğer hastalığı olmayan kişilerde intraplevral boşlukta hava birikmesi durumuna, primer spontan pnömotoraks (PSP) olarakadlandırılır. Onaltı yaşında erkek hasta 3 gün önce başlayan nefes darlığı ve göğüs ağrısı şikâyeti ile başvurdu. Dinlemekle sağ akciğerde solunum sesinin şiddeti azalmıştı. Hastanın çekilen akciğer grafisinde sağda pnömotoraks saptanması üzerine hastaya göğüs tüpü takıldı. Anamnezinde, 2 ay önce nefes darlığışikâyeti ile doktora başvurduğu, akciğer grafisi ve muayene bulgularına göre patolojik bir durumun olmadığı söylenmiş ve nefes darlığı 1-2 hafa içinde zamanla kaybolmuş. Hastanın 2 ay önceki grafisi retrospektif olarak incelendiğinde bilateral PSP olduğu görüldü. Spontan düzelmesi olan bilateral PSP vakasının nadirolması ve literatüre katkı sağlayacağı düşüncesi ile sunuldu. Göğüs ağrısı ve nefes darlığı ile kliniğe başvuran hastalarda ayırıcı tanıda pnömotoraks ın düşünülmesi gerekmektedir.

Abstract

A condition of intrapleural air-space accumulation in individuals without any history of trauma or lung disease is called as primary spontaneous pneumothorax(PSP). Sixteen-years-old male patient admitted with complains of chest pain anddyspnea beginning 3 day ago. On physical examination, severity of breath soundsdecreased on right side. Chest radiograph was taken and right-sided pneumotho- rax was detected and tube thoracostomy was inserted. Two months ago the patient referred to a doctor with similar complaints and physical examination andchest radiograph were reported as normal. The radiograph was retrospectivelyexamined and bilateral PSP was detected. We presented the case duo to sponta- neous recovery of bilateral PSP is seen very rarely and so contributes data to theliterature. In patients admitted to the clinic with chest pain and shortness of bre- ath, pneumothorax should be considered at diferential diagnosis.
