Özet
Renal transplantasyon, son dönem böbrek yetmezliğinde en iyi tedavi seçeneğidir. Renal transplantasyonda, kadaverik yada canlı donör nefrektomi sırasında gelişen renal ven hasarı yada kısalığı transplantasyon cerrahları içinteknik zorluk yaratır. Zaten var olan özellikle kadaverik donör azlığı nedeniyle greftin kazanılması ayrı bir önem arzeder. Bu yazıda Anastomoz yapılamayacak şekilde hasar görmüş renal venin, kadaverik vena kava graftı kullanılarak anastomoza uygun hale getirilmesi ve graft kaybının önlenmesi için klinik deneyimimizi paylaşmak amaçlanmıştır. Kadaverik vena kava graftı kullanarak onarılan ve anastomoz yapılabilecek uzunluğa getirilen renal ven, alıcının eksternal iliyak venine uç yan olarak başarıyla anastomoz edildi. Vasküleranastomozlar, teknik olarak kolayca uygulandı. Sıcak iskemi süresi 2 saat altındaydı ve post-operatif dönemde böbrek fonksiyoneldi. Renal ven trombozu gözlenmedi.Gerek kadaverik gerekse canlı donör nefrektomi sırasında oluşan renal ven hasarı çeşitli yöntemlerle onarılabilir. Bu damar onarımı yapılan böbreklerde, akut tubuler nekroz ve geç fonksiyon daha çok görülebilmesine rağmen başarı oranları yüksektir.
Abstract
Kidney transplantation is the best treatment choice in the end-stage renaldisease. In the renal transplantation, renal vein damage or shortness whichoccurs during cadaveric or living donor nephrectomy causes technical dif- ficulties for surgeons. The lack of the donors already especially cadaveric,the acquirement of the graft, gets very much importance. In this report, itis aimed to share the clinical experiment by which it seen, how anastomosiscan become appropriate by using the renal vein which is damaged in the waythat anastomosis cannot be done anyway by using cadaveric vena cava graft.The renal vein brought to length for anostomosis which is repaired by usingcadaveric vena cava graft, is anastomosed successfully by becoming an end- to-side of the external iliac vein of the recipient. Vascular anastomoses areapplied easily in technique. The time of the warm ischemia was under 2 hoursand the kidney was functional in the post-operative period. Renal vein trombosis was not observed. The renal vein damage occured during cadaveric orliving donor nephrectomy, can be repaired by some methods. In the kidneysin which vein requirement is done, the success rates are rather high althoughacute tubular necrosis and delayed function can be seen more.
