Gebelik sırasında saptanan adneksiyal kitle olgularında maternal ve fetal sonuçlar

Özet
AMAÇ: Gebelik kontrolleri sırasında adneksiyel kitle tespit edilerek takip ve tedavi edilen olgularda maternal ve fetal sonuçların değerlendirilmesi amaçlandı. MATERYAL ve METOD: Aralık 1994 ile Temmuz 2002 tarihleri arasında Yüzüncü Yıl Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Kliniğinde gebelik takipleri sırasında adneksiyal kitle tanısı konulan 19 hasta oran, gebelik haftası, ultrasonografi bulguları, biyokimyasal parametreler, tanı, uygulanan tedavi ile takipleri sonucunda gebelik prognozu açısından değerlendirildi. BULGULAR: Gebelik sırasında adneksiyal kitle tespit edilen 19 olgunun bu dönemde takip edilen gebelere oranı % 0.39 (19/4803) olarak tespit edildi. Olguların yaş ortalaması 29.41±6.24, gebelik haftası ortalaması 23.11±13.75 olarak saptandı. Kitlelerin çapları 5 ile 12 cm arasında olup 12 olguda 6 cm'den büyük kitle tespit edildi. Basit kist özelliğinde ultrasonografik bulguları olan 6 (% 31.58) olguda spontan takip sonrasında gerileme izlenirken, 13 (% 68.42) olgu operasyona alındı. 8 (% 42.11) olguda basit kist, 4 (% 21.05) olguda seröz papiller kist adenom, 3 (% 15.79) olguda matür kistik teratom, 2 (% 10.53) olguda myoma uteri, 1 (% 5.26) olguda pelvik kist hidatik ve 1 (% 5.26) olguda heterotopik gebelik saptandı. Heterotopik gebelik olgusuna 1. trimestirde laparoskopik salpenjektomi yapıldı ve takip sırasında 14. haftada ölü fetus saptanarak gebelik sonlandırıldı. Diğer olgularda fetal kayıp gözlenmedi. 8 cm boyutunda kitle ve akut batın semptomları nedeniyle 2. trimestirde laparotomi yapılan 1 olguda 34. haftada erken doğum eylemi gelişti. 5 olgu sezeryan, 14 olgu ise normal vajinal yolla doğum yaptı. SONUÇ: Altı cm'den küçük komplike olmayan kistlerin spontan olarak gerilediği görüldü. Adneksiyal kitle tanısı alan 2 olguda ise myoma uteri saptandı. Solid karakterdeki kitlelerin myom ile ayrımı konusunda dikkat edilmelidir. Adneksiyal kitlelerde malignite ihtimali göz önünde bulundurulmak koşulu ile gebeliği komplike etmediği sürece terme kadar beklenebilmektedir.
Abstract
OBJECTIVE: To determine maternal and fetal outcome in pregnant women with adnexal masses. STUDY DESIGN: 19 pregnant women who were diagnosed to have adnexal masses in Yuzuncu Yil University School of Medicine, Department of Obstetrics and Gynecology, were analyzed for their ultrasonographic and biochemical parameters, treatment modalities, and fetalmaternal outcomes at the end of their follow up. RESULT(s): The incidence of adnexal masses in pregnant women was 0.39 % (19/4803) during the 8 years of period. Mean age and gestational week were 29.4±6.2 years, and 23.1±13.8 weeks, respectively. Masses had a varying diameter from 5 to 12 cm and in 12 of the cases; it was larger than 6 cm. In 6 of the cases with sonographically simple cysts, spontaneous regression was observed. Thirteen cases (66.7 %) were operated. Simple cyst in 8 cases (42.1%), serous papillary cystadenoma in 4 cases (21.1 %), mature cystic teratoma in 3 cases (15.8 %), uterine fibroids in 2 cases (10.5 %), pelvic hydatid cyst in one case (5.3 %) and heterotopic pregnancy in one case (5.3 %) were found, respectively. Salpingectomy was performed by laparoscopy in one heterotopic tubal pregnancy case that had 5 cm adnexal mass in the first trimester and subsequently, intrauterine pregnancy was terminated at 14 weeks of gestation because of fetal demise. Fetal loss was not observed in other cases. One case with a mass of 8 cm underwent laparotomy at the second trimester, resulting in premature labor at 34th gestational week. Five cases were delivered by cesarean section, whilst remaining 14 cases were delivered by normal vagina l route. CONCLUSION(s): Simple non complicated adnexal masses which are less than 6 cm in diameter may regress spontaneously during pregnancy. In the differential diagnosis of solid adnexal masses, uterine fibroids should be sought thoroughly. Pregnancy may be followed up as long as there is no complication related to adnexal masses and malignancy should be ruled out completely.
