Özet

AMAÇ: Anormal uterin kanamaların etyopatolojisinin anlaşılmasında sonohisterografinin (SHG) tanı değerinin ortaya koyulması ve bunun diğer yöntemlerle karşılaştırılması. MATERYAL ve METOD: Anormal uterin kanama şikayetiyle başvuran 45 hasta pelvik muayene, ultrasonografi ve sonohisterografileri yapılarak incelendi ve hepsine tam küretaj uygulandı. Saptanan patolojik bulgulara göre histeroskopi veya histerektomi gerçekleştirildi. SHG'nin diğer tanı metodlarıyla (ultrasonografi, tam kü-retaj, histeroskopi, histerektomi) karşılaştırıldığında ortaya konan sensitivite, spesifisite, pozitif ve negatif belirleyicilik değerleri belirlendi. BULGULAR: Anormal uterin kanama şikayeti ile başvuran hastalardan 15'İ, fertil dönemde, 21'i perimenopozal ve 9'u postmenopozal dönemdeydi. Bu hastalarda USG, SHG, Histeroskopi ile TAH+BSO ve tam küretaj sonuçları birlikte değerlendirildiğinde % 46 endometriyal polip, % 17 basit endometriyal hiperplazi, % 13 gestagen etkiside endometriyum, % 13 östrojen etkisinde endometriyum, % 6 atrofik endometriyum, % 2 submüköz miyom saptandı. SHG'nin anormal uterin kanamaların etyolojisinin belirlenmesindeki sensitivitesi, spesifisitesi, pozitif ve negatif belirleyicilik değerleri sırasıyla % 96.6, % 86.6, % 93.5 ve % 92.8 bulunmuştur. SONUÇ: SHG, özellikle anormal uterin kanamalarda, uterin kaviteyi değerlendirmede oldukça yüksek sensitivite, spesifisite ve doğruluğu olan bir görüntüleme yöntemidir. Anormal uterin kanamalarının araştırılmasında etkin bir yöntem olmasının yanında, daha sonra uygulanacak cerrahi yaklaşımın seçilmesinde de yol gösterici olmaktadır.

Abstract

OBJECTIVE: To investigate the role of sonohysterography (SHG) in the diagnosis and assessment of abnormal uterine bleeding and to compare this with other diagnostic techniques. STUDY DESIGN: Following pelvic examination, ultrasonographic and sonohysterographic investigations of 45 patients with abnormal uterine bleeding, dilatation and curettage was performed to each patient. Thereafter, hysteroscopy or hysterectomy was performed according to the histopathologic findings. The sensitivity, specificity, positive and negative predictive values of SHG for differentiating the etiology of abnormal uterine bleeding in comparison to other diagnostic methods (USG, dilatation and curettage, hysteroscopy, hysterectomy) were determined. RESULT(s): Among patients with abnormal uterine bleeding, 15 women were at fertile age, 21 were in perimenopausal, and 9 were in postmenopausal period. Following diagnostic techniques, endometrial polyp was detected in 46 % of the patients, simple endometrial hyperplasia in 17 %, endometrium under the effect of gestagen in 13 %, endometrium under the effect of estrogen in 13 %, atrophic endometrium in 6 % and submucous fibroid in 2 % of the cases, were detected respectively. The sensitivity, specificity, positive and negative predictive values of SHG for differentiating the etiology of abnormal uterine bleeding were found to be 96.6 %, 86.6 %, 93.5 %, and 92.8 %, respectively. CONCLUSION(s): SHG is a valuable method for assessment of the uterine cavity and has high sensitivity and specificity for diagnosing patients with abnormal uterine bleeding. This effective method for investigating abnormal uterine bleeding is also very helpful for choosing the operation type which will be performed later.
