Özet

AMAÇ: Çalışmanın amacı, HELLP sendromlu hastalarda trombosit miktarı ile prognostik faktörler ve laboratuvar sonuçları arasındaki ilişkiyi incelemektir.MATERYAL ve METOD: Ocak 2001 ve Haziran 2004 arasında takip edilen HELLP sendromlu hastalar değerlendirildi. Hastalar trombosit miktarına göre sınıflandırıldı; trombosit sayısı < 50.000/mm3 olanlar Grup 1, 50.000 ile 100.000/mm3 arasında olanlar Grup 2, 100.000 ve 150.000/mm3 arasında olanlar Grup 3'ü oluşturdu.BULGULAR: HELLP sendromu 58 kadının 47'sinde (% 81) antepartum, 11'inde (% 19) postpartum, % 17'sinde 28. gebelik haftasından önce gelişti. Hastaların yaş ortalaması 28'di. Çoğu hasta (% 62.1) multipardı. Eklampsi vakaları 25 yaş altındaki HELLP sendromlu hastalarda preeklampsi vakalarından daha fazlaydı (p< 0.005). Maternal ve perinatal mortalite oranları sırası ile % 3.4 (iki hasta) ve % 32.8 (19 bebek)'di. Çalışma grupları arasında şiddetli maternal komplikasyonlar ve laboratuvar bulguları açısından önemli bir fark yoktu. Ancak, Grup 2 ve Grup 3'teki MAP değerleri Grup 1'den önemli derecede yüksekti (p< 0.001).SONUÇ: Yirmibeş yaş altındaki HELLP sendromlu hastalarda eklampsi gelişimi daha fazladır. Trombosit sayısı kullanılarak yapılan sınıflama, maternal ve perinatal sonuçları belirlemede yetersizdir.

Abstract

OBJECTIVE: The aim of this study is to investigate the relationship between prognostic factors and laboratory findings, and the blood platelet count in patients with HELPP syndrome.STUDY DESIGN: The pregnancies complicated by HELLP syndrome managed between January, 2001 and June, 2004 were evaluated. Patients were classified on the basis of the platelet count, with Class 1 defined as a platelet count < 50.000/mm3, Class 2 between 50.000 and 100.000/mm3, and Class 3 between 100.000 and 150.000/mm3.RESULTS: Of 58 women with HELLP syndrome; 47 (81 %) of the cases occurred antepartum and 11 (19 %) postpartum; 17 % developed at < 28 weeks. The median age of the patients was 28 years. Most of the patients (62.1 %) were multiparous. The cases of eclampsia were higher than preeclampsia in patients with HELLP syndrome who under age of 25 (p< 0.005). The rates of maternal and perinatal mortality were 3.4 % (two patients) and 32.8 % (nineteen babies), respectively. There were no significant differences in the severity of maternal complications and mortality, and laboratory findings between all study groups. But, MAP in Class 2 and Class 3 was significantly higher than Class 1 (p< 0.001).CONCLUSION: The development of eclampsia is higher in patients with HELLP syndrome under 25 years of age. The classification of the patients with HELLP syndrome on the basis of the platelet count is inadequate to determine the maternal and perinatal outcome.
