Özet

AMAÇ: Bu çalışma sezaryen yapılan olgularda genel ve spinal anestezi uygulamasının yenidoğan üzerine etkisini araştırmak üzere gerçekleştirilmiştir.MATERYAL ve METOD: Çalışmamız, Sağlık Bakanlığı Bakırköy Doğumevi, Kadın ve Çocuk Hastalıkları Eğitim Hastanesi Doğum Kliniği'ne Ağustos 2003-Nisan 2004 tarihleri arasında başvuran 62 gebeyi kapsamaktadır. Olgular, gebeliğin seyri açısından fetal ve maternal risk taşımayan, 37. gebelik haftasını doldurmuş, tekiz ve baş prezantasyonlu gebeliklerden oluşmaktadır. Rastgele 32 gebeye genel, 30 gebeye spinal anestezi uygulandı. Tüm olgularda aynı sezaryen tekniği uygulandı. Yenidoğanların APGAR puanı ölçümü çocuk hekimi tarafından yapılıp, hemen bebek çıkımında umbilikal arterden kan alınarak pH, pO2, pCO2 ve bikarbonat (HCO3) ölçümleri yapıldı. İstatistiksel hesaplamalarda bağımsız gruplar için Student's t testi kullanıldı. p< 0.05 değeri istatistiksel olarak anlamlı kabul edildi.BULGULAR: Genel ve spinal anestezi grubunda yaş, parite ve preoperatif hemoglobin, hematokrit değerleri açısından fark yoktu. Araştırılan parametrelerden 1. dk. APGAR puanı genel anestezi grubunda 7.88±0.6 olmasına karşın, spinal anestezi grubunda 8.03±0.6 idi (p= 0.352). 5. dk. APGAR puanı ise, genel anestezi grubunda 7.56±0.4 olmasına karşın, spinal anestezi grubunda 7.98±0.5 idi (p=0.387). Kan gazları analizlerinde ise, ortalama pO2 değerleri genel anestezi grubunda 26.15±8 mmHg'ya karşın, spinal anestezi grubunda 23.59±7 mmHg idi (p=0.188). Ortalama pCO2 genel anestezi grubunda 49.17±6.3 mmHg'ya karşın, spinal anestezi grubunda 50.36±8.9 mmHg idi (p=0.549). Ortalama HCO3 değerleri genel anestezi grubunda 23.73±1.1 mEq/L olmasına karşın, spinal anestezi grubunda 24±1.6 mEq/L olarak bulundu (p=0447). Umbilikal arter pH değerleri genel anestezi grubunda 7.308±0.02'yken, spinal anestezi grubunda 7.309±0.02 idi (p=0.998). Genel ve spinal anestezi gruplarında değerlendirilen tüm parametreler için istatistiksel anlamlı fark yoktu.SONUÇ: Elektif sezaryen operasyonlarında spinal ve genel anestezinin yenidoğana etkileri bakımından birbirlerine üstün olmadığı görülmüştür.

Abstract

OBJECTIVE: This study was performed to compare the effects of general and spinal anesthesia on newborns delivered by cesarean section.STUDY DESIGN: Our study comprises 62 pregnant women referred to our obstetric clinic between August 2003 and April 2004. All pregnancies were singleton, in 37th gestational age ,without any maternal and fetal risks, and vertex presentation. Randomly selected 32 women were practiced general anestesia and for 30 of them the spinal anesthesia were applied. Same operation technique was performed in all cesarean operations. Pediatrician determinated 1st and 5th minute APGAR values. Umblical cord samples were taken immediately after the child birth for the evaluation of pH, pO2, pCO2 and bicarbonate analysis. Student's t test was used for the statistical analyses and p< 0.05 was accepted statistically significiant.RESULT(s): There wasn't any significiant difference between the two groups for age, parity and preoperative hemoglobin and hematocrite values. Although the 1st. minute mean APGAR score was the 7.88±0.6 in the general anestesia group, it was 8.03±0.6 in the spinal anesthesia group (p=0.352) respectively. The 5. minute mean APGAR score in the general anestesia group was the 7.56±0.4 however in the spinal anestesia group it was 7.98±0.5 (p=0.387) respectively. According to the blood gas analyses, in the general anestesia group, the mean pO2 value was 26.15±8 mmHg vs. It was 23.59±7 mmHg in the spinal anestesia group (p=0.188) respectively. The mean pCO2 values in the general anetesia and spinal anestesia groups were 49.17±6.3 mmHg vs 50.36±8.9 mmHg (p=0.549) respectively. The mean HCO3 values in the general anetesia and spinal anestesia groups were 23.73±1.1 mEq/L vs 24±1.6 mEq/L (p=0.447) respectively. The mean umbilical artery pH values in the general anetesia and spinal anestesia groups were 7.308±0.02 vs 7.309±0.02 (p=0.998) respectively. There was no statistically significiant difference for all the parameters between the two groups.CONCLUSION: In elective cesarean operations, there's no superiority of general and spinal anesthesia against each other for effects on newborns.
