İnfertil hastalarda gonadotropin ile ovulasyon indüksiyon protokolüne, gonadotropin realising hormon antagonisti ilavesinin etkileri

Özet
Amaç: Kontrollü overyan stimulasyon ve intrauterin inseminasyon planlanan hastalarda, ovulasyon indüksiyon protokolüne gonadotropin realising hormaon antagonisti (GnRHant) ilavesinin siklus ve gebelik oranları üzerine etkisinin araştırılması. Materyal ve Metod: Çalışmamız randomize kontrollü çalışma olup Mayıs 2010 ile mayıs 2011 tarihleri arasında Sağlık Bakanlığı Bakırköy Kadın Doğum ve Çocuk Hastalıkları Eğitim Araştırma Hastanesi Etik Kurulunun 12.06.2009 tarihli ve 254 sayılı kararı ile başlatılmıştır. Güncel verilere ve tanı kriterlerine dayanılarak (RCOG-Rotterdam 2003) açıklanamayan infertilite ve polikistik over sendromu (PKOS) tanısı alan 47 hastada 56 siklus incelenerek çalışma yapıldı. Tüm hastalara siklusun 2-4. gününde düşük doz step up protokolüne göre rFSH ile ovulasyon indüksiyonu ile tedaviye başlandı. Antagonist grubunda 25 hastaya 30 siklusda rFSH’a, önde giden folikül çapı 14 mm’ye ulaştığında tedaviye 0.25 mg/gün GnRHant eklendi. Kontrol grubunda 22 hastaya 26 siklusda yalnızca rFSH kullanılarak düşük doz step-up protokolü ile ovulasyon indüksiyonu sonrası önde giden folikül 18 mm’ye ulaştığında 250 mcg rekombinant hCG enjeksiyonu yapıldı ve bundan 36 saat sonra İUİ uygulandı. Bulgular: İndüksiyon gün süresi, kullanılan ortalama rFSH miktarı (IU), folikül sayısı, hCG günü E2 düzeyi ve endometrial kalınlık açısından iki grup arasında istatistiksel olarak anlamlı bir farklılık gözlenmedi. Antagonist ilave edilen grupta erken LH piki % 3.3 ve kontrol grubunda % 34.6 saptanmış olup antagonist grubunda erken LH piki anlamlı olarak (P 0.002) daha az görüldü. Siklus başına gebelik oranları, antagonist grubunda % 10 ve kontrol grubunda %7.7 olup iki grup arasında istatiksel olarak anlamlı fark bulunmadı (P 0.763). Sonuç: Gonadotropinlerle yapılan ovulasyon indüksiyon protokolüne GnRHant eklemenin, erken LH pikini anlamlı olarak azalttığı ama gebelik sonuçları üzerine bir etkisi olmadığı görülmüştür.
Abstract
Objective: The aim of this study is to invastigate the cycle outcomes and pregnancy rates in infertile patients treated with GnRH antagonist added gonadotrophins followed by intrauterin insemination (IUI). Materials and Methods: This is a randomised controlled trial starting from may 2010 to may 2011, with the verdiction of Health Ministery Bakırkoy Gynecology and Obstetrics and Pediatrics Education and Research Hospital Ethics Commitee dated 12/06/2009 and numbered 254. We analysed the data on IUI cycles starting from may 2010 to may 2011 in 56 cycles of 47 patients diagnosed with unexplained infertility and polycyctic ovary syndrome (PCOS) with current guidelines (RCOG and Rotterdam Concensus 2003). All patients were treated with recombinant FSH in a low dose step up regimen starting on day 2-4. 25 women in the antagonist group, 0,25 mg per day GnRH antagonist was added when leading follicul reached the size of 14 mm in 30 cycles. In the control group there were 22 women treated with only recombinant FSH in 26 cycles. When at least one follicle reached a size of ≥18 mm, ovulation was induced by hCG injection. IUI was performed 36 hours later. Results: rFSH amount, number of follicules, E2 level in the hCG day and endometrial thickness were not significantly different between the antagonist group and the control group. Also there was statistically no difference in the induction period time. Mean early LH peak was observed in %3.3 of cases of antagonist group, while it was %34.6 in the control group, showing that the early LH peak was statistically lower in the antagonist group (p:0.002) but there was no statistical difference in the pregnancy rate for cycle between the two groups, as %10 in the antagonist and %7.7 in the control group (p:0.763). Conclusion: Adding GnRH antagonist in cycles with rFSH followed by IUI has significant effect on early LH surge, but it does not increase pregnancy rates.
