Çok düşük doğum ağırlıklı prematürelerde erken yoğun parenteral beslenmenin erken dönem prognoza etkisi

Özet
Amaç: Geç dönemde düşük protein ve lipit ile beslenen çok düşük doğum ağırlıklı prematürelerle erken dönemde yüksek protein ve lipit başlanan aynı özellikteki prematürelerin prognozunu araştırmaktı. Yöntemler: Çalışmaya 1500 gr altında doğan prematüreler alındı. Protein desteği ilk 48 saatten sonra, lipit desteği 7. günden sonra total parenteral sıvısına eklendi. Bu beslenmeye “Geç Parenteral Beslenme” adı verildi. Grup II’deki olgular ilk 24 saatte 2 gr/kg/gün protein başlanıp, ikinci gün 3.5 gr/kg/güne çıkılan, lipit komponenti ise 24-48 saatte 1 gr/kg/gün başlanarak hızla 4 gr/kg/güne ulaşan prematürelerdi. Bu parenteral beslenme şekline “Erken Yoğun Parenteral Beslenme”adı verildi. Her iki gruptaki bebeklerin gebelik haftası (GH), doğum ağırlığı (DA), cinsiyet, doğum şekli, parenteral beslenme zamanı, protein ve lipit başlanma zamanları, kalsiyum- fosfor desteği alıp almadıklar, parenteral beslenme süreleri, enteral beslemeye başlama yaşı, doğum ağırlığını yakalama günü, anne sütü zenginleştirici alıp almadıkları, yatış süreleri ve taburculuk sırasında ağırlık, boy ve baş çevre özellikleri kaydedildi. Bulgular: Gruplar arasında doğum şekli, doğum ağırlığı, cinsiyet açısından istatistiksel fark yoktu. Grup I’deki % 46 bebeğe, grup II’de % 89,5 bebeğe lipit kullanıldı. Doğum kilosuna ulaşma süresi ve enteral beslenme başlangıç zamanı, tam enteral beslenmeye geçiş süresi, kalsiyum, alkalen fosfataz, protein, albumin ve trigliserit düzeyleri arasında gruplar arasında belirgin istatistiksel fark saptandı. Abstract
Objective: To compare the prognosis in very low birth weight preterms being started late onset low protein and lipid feeding with early onset high protein and lipid feeding. Methods: Group 1 consisted of preterms with parenteral protein support after 2 days. This type of feeding was named as ‘late parenteral feeding’. In Group 2, protein supplement was started as 2 gr/kg/d in the first 24 hrs and increased to 3.5 gr/kg/d in the 2nd day, lipids were started as 1 gr/kg/dy in the first 24-48 hrs and increased to 4 gr/kg/d rapidly. This type of feeding was named as ‘early intense parenteral feeding’ Gestational weeks (GW), birthweights (BW), gender, timing, and duration of parenteral feeding, age at initiation of enteral feeding, catch-up time of age-matched birthweight, intake of breast milk enriching supplements, hospital stays, weight, height, and head circumference of newborns at discharge were recorded. Results: 125 preterms were evaluated in Group 1 and 151 preterms in Group 2. There was no statistically significant intergroup difference regarding the method of delivery, birth weights and gender. Lipid supplements were given 46 % of the newborns in Group 1 and 89.5 % of Group 2. The catch-up time of birth weight, age at the begining of enteral feeding, timing of full enteral feeding, levels of calcium, alkaline phosphatase, protein, albumin and triglyceride levels were significantly different between groups. Duration of hospital stay, height, weight and head circumferences during hospital discharge were similar between groups. Conclusion: Early intense parenteral feeding is especially very important for low birth-weight (< 1500 g) infants. The catch-up time of birth weight and time to switch to full enteral feeding were significantly short in the second group.
